About this file
This is a Responsibility Inquiry form (Attachment E) issued by the Oregon Department of Administrative Services (DAS) Procurement Services for RFA #S-10000-00016919, an on-call feeding and bottled water delivery service contract. The form establishes the evaluation criteria and documentation requirements that applicants must complete to demonstrate financial, operational, and legal responsibility prior to award and execution of a price agreement. Applicants must certify their capability to meet all contractual responsibilities, including availability of appropriate financial, material, equipment, facility, and personnel resources. The agency reserves the right to request additional documentation beyond this form, including profit-and-loss history, balance statements, cash flow information, assets-to-liabilities ratios, bonding capacity, credit information, facility capabilities, personnel information, and records of past contract performance.
The form requires applicants to disclose their contracting history over the past five years, including the number of similar contracts completed on time and within budget, as well as any contracts that failed to meet performance standards. Applicants must also declare any criminal or civil charges, indictments, or convictions within the past three years involving public contracts, antitrust violations, or crimes such as embezzlement, bribery, or falsification of records. Additional responsibility questions address contract terminations for default, pending lawsuits, outstanding judgments, financial distress, bankruptcy filings, and whether the applicant possesses all required licenses, certifications, insurance, and legal authorization to conduct business in Oregon. For contracts exceeding $500,000 or applicants employing 50 or more full-time workers, a current Pay Equity Compliance Certificate issued by the Oregon Department of Administrative Services is required. Failure to promptly provide requested information or demonstrate responsibility may result in a finding of non-responsibility and rejection of the application.
