About this file
This document is an Attachment E - Responsibility Inquiry for a state and local contract opportunity issued by the Agency for a Hygiene Services Price Agreement. The purpose of this Agreement is to pre-negotiate terms so that the Agency can procure hygiene services for its clients during emergency or disaster events, extreme weather incidents, or other situations where the Agency is responsible for providing human services. The Agency will evaluate the responsibility of the applicant prior to award, which may include reviewing the applicant's financial, material, equipment, facility, and personnel resources and expertise, as well as the applicant's past performance on similar contracts. The applicant must also have all required licenses, certifications, insurance, and registrations to legally do business in the state of Oregon.
The Price Agreement will be awarded through a Request for Applications (RFA) process. The applicant must provide information regarding any criminal or civil charges, contract terminations, lawsuits, financial distress, and bankruptcy filings within the past three years. The applicant must also possess a valid Pay Equity Compliance Certificate, if applicable. The Price Agreement resulting from this RFA will have pre-negotiated terms to allow the Agency to quickly procure hygiene services as needed during emergency or disaster events.
