About this file
This document is an Attachment A - Application and Certification form for an RFA (Request for Applications) issued by the Oregon Department of Human Services (ODHS) and its Office of Vocational Rehabilitation (VR) for Independent Hearings Officer Services. The RFA S-10000-00009665 is seeking qualified applicants to serve as Impartial Hearings Officers to resolve disputes between VR and its applicants, clients, and former clients. Applicants must provide two references, agree to the contract terms, and certify compliance with various state requirements.
The RFA does not provide specific details on pricing, set-asides, incumbents, or funding sources. However, it outlines the application and certification requirements for interested parties, including that the anticipated contract price must be $150,000 or more, and that the selected contractor must comply with state laws regarding wage discrimination, broadband internet access, and conflict of interest.
