About this file
The provided document is Attachment C - Applicant Questionnaire and Service Selection, which is part of a Request for Applications (RFA) issued by an agency in the state of Oregon for an On-Call Feeding and Bottled Water Delivery Service contract. The RFA seeks qualified applicants to provide and deliver meals, food boxes, bottled water, and meal cards/vouchers on an on-call basis to the agency's clients throughout the state of Oregon who have been affected by a declared emergency, disaster event, extreme weather incident, or other incidents requiring mass care support services. The applicants can select to provide large-scale meal preparation, small-scale meal preparation, on-site meal preparation, food box preparation and delivery, and/or bottled water services. Applicants must specify the Oregon counties they wish to serve and can negotiate service rates with the agency.
The document outlines the pricing terms and service rates for the various feeding and bottled water services. Applicants must provide their current service rates, which must be all-inclusive of travel costs, fuel, delivery charges, and meal packaging. The agency requires applicants to indicate if they are willing to negotiate the provided service rates. The contract does not specify any set-asides, incumbents, or funding requirements and sources.
