About this file

This is a Responsibility Inquiry form (Attachment F) from the State of Oregon's Department of Administrative Services related to RFP #S-12300-00010481 for Furthering Fair Housing Activities 2024. The form serves as a pre-award responsibility determination document that proposers must complete to demonstrate their capability to meet all contractual obligations. The form does not specify a particular response deadline, site visit requirements, or bidder's conference details, as it functions as a supplementary responsibility assessment tool rather than a primary solicitation document. Proposers must certify their financial, material, equipment, facility, and personnel resources through a series of yes/no questions and detailed explanations where applicable. The form requires disclosure of contract performance history over the past five years, including the number of contracts completed within time and budget constraints, as well as any contracts that failed to meet those standards.

The Responsibility Inquiry form addresses critical evaluation criteria including criminal or civil charges against the proposer or major subcontractors within the past three years, contract terminations for default, pending litigation or judgments, financial distress or cash flow concerns, bankruptcy filings, required licenses and insurance, and legal authorization to conduct business in Oregon. For proposers employing 50 or more full-time workers with a prospective contract price exceeding $500,000, a current Pay Equity Certificate from the Department of Administrative Services is required, with specific exemptions for architectural, engineering, photogrammetric mapping, transportation planning, and land surveying services. The form does not contain pricing information or set-aside designations, as these details would be specified in the primary RFP documentation. All responses must be certified by an authorized representative of the proposer as complete, accurate, and not misleading.
