About this file
This document is a supplemental terms and conditions attachment related to dock leveler services being procured by the Illinois Department of Human Services (DHS) for the Elisabeth Ludeman Center located in Park Forest, Illinois. The attachment establishes compliance requirements for vendors providing services on this public works project, including mandatory adherence to Illinois labor regulations, prevailing wage standards, and workplace safety protocols. Vendors must employ a minimum of 90% Illinois laborers on the project during periods of excessive unemployment (defined as any month following two consecutive months when Illinois unemployment exceeds 5%), with exceptions available only through written certification demonstrating that Illinois laborers are unavailable or incapable of performing specific work types. All exceptions must be submitted within the first quarter of the contract term and require agency approval.
The vendor must certify compliance with the Illinois Prevailing Wage Act and the Illinois Procurement Code, committing to pay all laborers, workers, and mechanics no less than the current prevailing wage rates established by the Illinois Department of Labor for the county where work is performed. Vendors are responsible for obtaining current prevailing wage rate information from the Illinois Department of Labor website and monitoring updates throughout the contract period. The document requires vendors to demonstrate competence and experience, maintain employee identification, comply with OSHA standards, adhere to all traffic regulations and security protocols at the facility, and ensure all work meets quality standards with unsatisfactory work corrected at the vendor's expense. Responsive bids must include a checked "YES" box confirming compliance with these prevailing wage requirements.
