About this file
This document is an Application Response Form for the Refugee Health Assessment Program (RHAP) Request for Response (RFR) #253933 issued by the Commonwealth of Massachusetts Executive Office of Health and Human Services Department of Public Health. The RHAP seeks providers to deliver health assessment services to newly arrived refugees in Massachusetts. The response requires details on the applicant's organizational structure, staffing, operational workflows, capacity to deliver services, collaborations, and financial commitments. Responses must be submitted through the Commonwealth's online procurement system, COMMBUYS. The program uses a cost reimbursement payment model, and applicants must provide a detailed program budget. Applicants must also demonstrate compliance with federal and state disability non-discrimination laws and provide supporting documentation, including organizational charts, staff demographics, and subcontractor lists, if applicable.
The payment model has transitioned to cost reimbursement, and applicants must provide a detailed program budget using standardized line items. Applicants are required to make a meaningful financial commitment by partnering with certified diverse business enterprises or non-profit organizations, with a target of at least 1% of contract revenue directed to these partnerships. The program seeks to promote racial/health equity through applicants' current or planned operations.
