About this file
This document contains questions and answers related to Request for Response (RFR) 231332 for Sexual Assault Nurse Examiners (SANE) services in Massachusetts. The RFR is being issued by the Massachusetts Department of Public Health to procure SANE services for a new contract term starting on July 1, 2022. The RFR covers topics such as required documentation, immunization records, CORI forms, and the application process. Applicants who are currently contracted SANEs under RFR 302022 are eligible to apply for the new contract. The application can be started and completed at a later time, and no additional paperwork such as the standard contract form or W-9 is required to be submitted at this time.
Regarding pricing, the prompt pay discount form is optional, and applicants can choose to maintain the current 45-day payment processing time. The RFR does not mention any set-asides, incumbents, or specific funding requirements. Applicants are advised to submit all required documentation, and the SANE program will review submissions and respond to inquiries within 3 business days.

Questions & Answers
1/24/2022 Q1. Under immunizations, I included my marriage license to reflect my name change since some of my lab results were under my maiden name.
A. That is helpful and acceptable.
Q2. I also included 2 copies of my insurance information. I included the existing policy, as well as the one that renews in March 2022 under immunizations.
A. That is helpful and acceptable. Any required credentials – including professional liability insurance – which are current now but expire before 7/1/22 will need to be updated prior to contract initiation.
Q3. Do we also need to submit the standard contract form and W-9? I just submitted this to you recently. If so, I have questions about the standard contact inform, including if we indicate that this is a new contract, how to complete the anticipated start state, the business mailing address, MMARS Doc IDs, and contract end date fields.
A. No; the standard contract form is included in the attachments which are for information only. Upon selection for contracting until this RFR, a pre-filled Standard Contract Form will be provided to the contractor for signature.
The W9 is required only for new contractors (i.e. those who do not have a current/existing contract for SANE care), or for those contractors who have a current/existing contract but whom also need to update their name or address.
1/25/22 Q4. I am currently a contracted MA SANE. I filled out and submitted the online application, and I am reviewing the paperwork that needs to be mailed into the SANE department. I am unclear how to fill out the Vendor Activation Cover Page form. There is a section called "Organization Information." It is asking for the Organization name and FEIN. Is that your SANE department information? Can you please clarify how I fill this out.
A. None of the paperwork is required to be mailed; all required documentation may be uploaded via the online application. Note: the “Vendor Activation Cover Page” does say to mail some documents, but this can be disregarded. The Vendor Activation Cover Page is included in the VENDACT files, which means that it is a form to be complete only by applicants new to the state system. If you are a currently contracted MA SANE, you do not need to complete this form.
Q5. Am I currently contracted as a MA SANE vendor (under MA SANE RFR#302022)? This is my first time doing this process.
A. If you currently have a contract in place with DPH to provide care as a MA SANE, then you are currently contracted.
Q6. I have uploaded 2 pdf files, but when I try to open them to double check I get an error message. Is this just on my end?
A. To inquire about the status of a particular document upload, you may email MASANE@mass.gov with your name and the document in question; we will review your application and get back to you. Please allow 3 business days for response to questions related to specific document uploads.
Q7. Does SANE have our immunization records from our previous application? I am having trouble finding my older vaccines, such as varicella.
A. The MA SANE Program does maintain some documentation related to vaccination records for currently contacted SANEs. However, for the purposes of this RFR we are unable to provide that documentation back to contracted SANEs at this time. As this is a new procurement which will be initiating all new contracts, all documentation must be provided by the applicant.
1/26/22 Q8. Does the completion of this process have to be done in one sitting (as in- once you start you can't stop or else you'll have to start all over again). Or can you start process and go back to it once you're able to obtain the additional information needed to complete the process.
A. It is possible to start the submission, pause, and return to it later. When you navigate to the application page, you will see information related to how to pause your submission and return to it at a later time.
1/27/22 Q9. For the CORI form it is asking for an employee verification signature. How do we do this online if we need it verified by an employee?
A. On the CORI Form, the “Verifying Employee” can be left blank. Upon contract approval, we will reach out to contractors for verification/completion.
Q10. Under immunizations, it says we need "Record of a minimum of one negative TB-ppd/Mantoux" does it mater what date it is?
A. No, there are no specific requirements for the date of the TB-ppd/Mantoux; however, you should upload your most recent.
Q11. It also does not ask for flu vaccination or covid vaccinations and states " Please provide only the minimum necessary information." So, I am confused if we should submit those or just what they are specifically asking for.
A. Please do not include additional health or immunization information; documentation of flu and COVID vaccination are not required for this application.
Q12. Do I need to fill in and submit the Standard Contract Form?
A. No; please see above, Q3.
Q13. If my EFT isn't changing and my bank account information is already on file, do I need to submit that one again?
1. No; the EFT form is required only for new contractors (i.e., those who do not have a current/existing contract for SANE care), or for those contractors who have a current/existing contract but whom also need to update their EFT information.
Q13. Once I send everything in today, will someone be able to let me know if I am missing anything?
A. To inquire about the status of your application, you may email MASANE@mass.gov with your name and the date of submission; we will review your submission and get back to you. Please allow 3 business days for response to questions related to submissions.
Q14. I see the CORI needed for contracting. Do they leave the “signature of the Verifying Employee” blank?
A. Yes; see above, Q9
1/31/2022 Q15. I was just looking for clarification of how often we need a PPD test. My last one was 2020, is this a yearly requirement? I did not see it listed as yearly but did see it on the immunization requirements!
A. The online application says “Record of a minimum of one negative TB-ppd/Mantoux”; there is no specific date requirement for this immunization, but it is preferable that you submit your most recent.
Q13. Also do we need to send flu/ Covid vaccine info? I had sent this to my RC previously but can attach it to this as well!
A. No; See above, Q11
Q17. when scanning documents do you need all the pages scanned and uploaded or just the first page, initialed page and signature page? there are many documents with pages of print in between and it would be faster not to scan them.
A. Please include/upload all pages of required documents
Q18. Do I need to submit the prompt pay discount? I'd prefer payments as they have been but don't really understand how this works.
A. The Prompt Pay Discount (PPD) form is optional. If you prefer to maintain the current (45-day) processing time, you do not need to include the Prompt Pay Discount Form. If, on the other hand, you wish to receive your payments in less than 45 day and therefore would like to participate in Prompt Pay Discount, you will need to include the form. On the form, you will need to indicate the % discount you are offering. More details on PPD can be found on the PPD form, included in File Attachments on the Commbuys page.

Q19. Is our RFR# 231332? Because on the application that is what’s listed at the top, but then one of the questions reads: Are you CURRENTLY contracted with the Commonwealth as a MA SANE vendor (under MA SANE RFR#302022)?
A. RFR 302022 is RFR that MA SANEs are currently contracted under, and which expires 06/30/2022. RFR 231332 is the new RFR, for which SANEs are now applying, and under which contracts will begin 07/01/2022.
