All Interested Parties:

This Request for Quotation (RFQ) is to provide mental health and substance abuse treatment services for male and female offenders held under authority of the United States in Toledo, Ohio. Services are to be provided in accordance with the enclosed Statement of Work entitled Federal Bureau of Prisons, Community Transitional Drug

Abuse Treatment (TDAT) Services Northeast Region-No Master’s

Required, dated July 2010.

The following must be completed and submitted with your quotation:

Standard Form (SF) 1449 - Solicitation/Contract/Order for

Commercial Items - Please completes blocks 12, 17a, 17b, 23, 24, 30a, 30b, and 30c, of the SF-1449 and the continuation sheets. Please include your telephone number in Block 17a.

Original signatures are required.

Representations and Certifications (FAR 52.212-3) -

Prospective quoters are required to submit Annual

Representations and Certifications via the Online

Representations and Certifications Application (ORCA) website at http://orca.bpn.gov. By submission of a quote under this solicitation, the quoters acknowledges completion of the annual representations and certifications via ORCA and verifies, by the submission of the quote, that the representations and certifications currently posted in ORCA were entered or updated within the last 12 months and are current, accurate, complete and applicable to this solicitation. All quotes must be capable of and comply with FAR 52.212-1(j), Instructions to

Offerors - Commercial Items, Data Universal Numbering System

(DUNS) Number (JUNE 2008) and FAR 52.212-4(t), Contract Terms and Conditions - Commercial Items, Central Contractor

Registration (CCR) (JUNE 2010). Submission of your quote constitutes compliance.

The Government shall award to the lowest priced, technically acceptable, responsive and responsible quoter. Award will be acceptance of your quote, therefore, ensure that all requirements can be satisfied.
