QUESTIONS
The following questions and answers are provided for informational purposes only. Nothing contained in the questions or answers constitutes a change to the terms and conditions of the solicitation. Any change to a term or condition will be issued through an amendment to the solicitation.
1.
Will the optometrist/ophthalmologist (Section III.4.0 p. 41 of SOW) be subject to the National Agency Check mentioned on p. 84 in Section VII.5.0?
No. The optometrist/ophthalmologist is not considered to be a “hands on” physician; therefore, he/she will not be subject to the National Agency Check.
2.
Please clarify what fields on the FAA Form 8500-7 are to be included in the “full eye examination” performed by a licensed optometrist or ophthalmologist (Section Iii.4.0.D on p. 41).
All fields on the FAA Form 8500-7 are to be included in the full eye examination performed by a licensed optometrist or ophthalmologist.
3.
Attachment 28 (p.1) lists the requirements of our submission(s) to include “(4) copies” of disk one and “(1) Copy” of Disk Two. Please clarify whether this refers to the total number of copies of each CD (and its contents) or the total number files to be included on each CD?
Quotes should consist of four individual volumes/sections of information and be submitted electronically on compact disk (CD). (4) Copies of Disk One should be submitted which should contain: Volume 1) Required Credentials/Licenses/Insurance; Volume 2) Technical Proposal; Volume 3) Past Performance Questionnaire. (1) copy of Disk Two should be submitted which should contain Volume 4 - price quote.
4.
If a line item is not available (such as an FAA Class I, II, III Examination or Opthalmology/Optometry Examination), what should be placed in the pricing matrix for those line items? Attachment 28 Section I.3 (p.3) states “line items that are not required and left blank may be viewed as Non Responsive.” Please clarify
Per the pricing matrix instructions, “Any line item that is left blank may be viewed as Non-Responsive. If a particular service is not available, please indicate N/A. If a line item is included in the price of another line item, please explain.”
5.
Where on the pricing matrix should we include the price for plasma cholinesterase, which appears to be a part of the hazardous materials response team examinations (SOW Section III.8.1.0 on p. 53)?
The price for item B.5, Hazardous Materials Response Unit (HMRU) Examination-Initial, should include the price for all of the required components as indicated on form FD-944, FBI Hazardous Material Response Team – Initial or Pre-Employment.
The price matrix also indicates a price for, “RBC Cholinesterase x 1 draw” at item 14.dd.
6.
If invoices are submitted electronically, is the contractor still required to submit three copies along with the original (SOW Section I.1.0.g. on p.2)?
Electronic invoicing is not authorized at this time. Please refer to Section VI.3.0 for invoicing requirements.
7.
Are FAA Class I, II, or III examinations required for any of the locations included in this solicitation? While Section III.10.0 (p.56) states Class II exams are not required, each pricing matrix marks each class as “not required.”
Section III.10.0 has been revised to clarify the requirement for FAA examinations. Please refer to an amendment including the revised Statement of Work.
8.
Section VII.5.0.d (p. 84) states that “ ‘hands on’ physicians and office personnel who have access to the records and administrative information” are required to complete a “National Agency Check (NAC)”. However, no further information is provided other than the need for fingerprints. Can you provide more specific information as to what the NAC entails and copies of all applicable forms? Additionally, how does the NAC differ from the “background investigation” mentioned in Section VII.5.0.a (p. 83), if at all?
The background investigation, which entails a National Agency Check (NAC), will be initiated upon contract award. The contractor will be required to complete security related forms. In addition, the following may be required: fingerprint cards, proof of legal U.S. status if the candidate is foreign born, and a consular report of birth form if the candidate acquired U.S. citizenship through birth abroad to a U.S. citizen(s) or a naturalized U.S. citizen(s).
The security forms applicable to the National Agency Check are attached and are subject to change.
The term background investigation mentioned in Section VII.5.0.a does not differ from the NAC.
