The NAICS code is 339114 (Dental Equipment and Supplies Manufacturing) with a small business size standard of 1,000 employees. All vendors must maintain active registration with the System for Award Management (SAM) prior to award. The government intends to award on an "All or None" basis to the lowest priced responsive and responsible offeror. Pricing will be evaluated in whole, including all optional accessories. Arizona state tax exemption applies. All invoicing must utilize the Invoice Processing Platform (IPP) in accordance with federal requirements. Indian Health Service (IHS) RFQ-26-PHX-034 TO RECEIVE UP TO DATE INFORMATION ON THIS REQUIREMENT, INCLUDING ANSWERS TO VENDOR QUESTIONS, MODIFICATIONS TO THE REQUIREMENT OR EXTENSIONS TO THE OFFER DUE DATE, USE THE FOLLOW THIS REQUIREMENT FEATURE TO ENSURE SYSTEM NOTIFICATIONS.
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. d Solicitation # RFQ-26-PHX-034 and this notice is issued as a Request for Quotation (RFQ). This is a combined synopsis/solicitation for commercial commodities/services. Submit written quotes only, oral offers will not be accepted. All firms or individuals responding must be registered with the System for Award Management (SAM).
This requirement is under North American Industrial Classification Standard (NAICS) codes: 339114 Dental Equipment and Supplies Manufacturing, Small Business Size Standard: 1000 employees. This requirement is set-aside 100% Buy Indian (specific to Department of Health and Human Services, Indian Health Services); wherein, Buy Indian means American Indian and Alaska Native Small businesses, in accordance with the criteria and size standards of 13 CFR Part 121.
Definitions. Indian Economic Enterprise (IEE). Any business activity owned by one or more Indians or Indian Tribes that is established for the purpose of profit provided that: the combined Indian or Indian Tribe ownership must constitute not less than 51 percent of the enterprise; the Indians or Indian Tribes must, together, receive at least a majority of the earnings from the contract; and the management and daily business operations of an enterprise must be controlled by one or more individuals who are Indians. To ensure actual control over the enterprise, the individuals must possess requisite management or technical capabilities directly related to the primary industry in which the enterprise conducts business. The enterprise must meet these requirements throughout the following time periods:
a. At the time an offer is made in response to a written solicitation;
b. At the time of the contract award; and
c. During the full term of the contract.
Indian Small Business Economic Enterprise (ISBEE). An IEE that is also a small business concern established in accordance with the criteria and size standards of 13 CFR part 121. Offerors that do not fall into this category will not be considered for award.
REQUIREMENTS: Indian Health Service (IHS) – Phoenix Area Office (PAO), 40 North Central Ave., Phoenix, Arizona 85004-4424 has a requirement for Panorex Dental Xray Unit for the Whiteriver Service Unit (WRSU).
This procurement is for NEW Equipment ONLY; no remanufactured or "gray market" items. Vendor shall be an Original Equipment Manufacturer (OEM authorized dealer, authorized distributor or authorized reseller for the proposed equipment/system such that OEM warranty and service are provided and maintained by the OEM. All, warranty and service associated with the equipment shall be in accordance with the OEM terms and conditions. All Equipment must be covered by the manufacturer's warranty. The quote MUST include a copy of the authorized distributor letter from the manufacturer to verify that the vendor is an authorized distributor of the products being quoted; failure to provide evidence of this may result in your proposal not being further considered.

This is an open-market combined synopsis/solicitation for products as defined herein. The government intends to award a purchase order as a result of this combined synopsis/solicitation to the lowest priced responsive and responsible offeror (LPTA) on an “All or None” basis, meaning that the offeror must be able to supply the total quantity requested of the acquisition to be considered for award.
This combined synopsis/solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2026-01 dated 03/13/2026. It is the contractor's responsibility to be familiar with the applicable clauses and provisions.

The provision at FAR 52.212-2, Evaluation – Commercial Items does not apply to this acquisition. This acquisition will be evaluated based on price. This acquisition will be awarded to the lowest priced responsive and responsible offeror on an “All or None” basis, meaning that the offeror must be able to supply the total quantity requested of the acquisition to be considered for award.
All offerors are advised to include a completed copy of the provision at FAR 52.212-3, Offeror Representations and Certifications – Commercial Items with their quote.
PRICING: Price will be evaluated in whole, including all optional accessories quoted.

INVOICING PROCESSING PLATFORM (IPP) (Feb 2022)
In compliance with the Office of Management and Budget (OMB) M-15-19 memorandum “Improving Government Efficiency and Saving Taxpayer Dollars Through Electronic Invoicing” directing Federal agencies to adopt electronic invoicing as the primary means to disburse payment to vendors. Invoices submitted under any award resulting from this solicitation will be required to utilize the Invoice Processing Platform (IPP) in accordance with HHSAR 352.232-71, Electronic Submission and Processing of Payment Requests”
IPP is a secure, web-based electronic invoicing system provided by the U.S. Department of the Treasury’s Bureau of the Fiscal Service, in partnership with the Federal Reserve Bank of St. Louis (FRSTL). Respondents to this solicitation are encouraged to register an account with IPP if they have not already done so. If your organization is already registered to use IPP, you will not be required to re-register- however, we encourage you make sure your organization and designated IPP user accounts are valid and up to date.

Note: IAW OMB memorandum M-21-07, The Office of Management and Budget (OMB) mandates that U.S. federal agencies use the Internet Protocol version 6 (IPv6). In November 2020, OMB issued memorandum M-21-07 "Completing the Transition to IPv6,” which outlines the Federal government's strategic intent "to deliver its information services, operate its networks, and access the services of others using only IPv6". The IHS requires all new IT systems (hardware and software) to operate in a pure IPv6 network environment. This includes on-premises services or third-party hosted services that require network integration into the IHS network.
