This is a Request for Quotation (RFQ) document issued by the Ohio Department of Natural Resources for Mosquito Lake State Park located in Cortland, Ohio. The project requires contractors to replace decking and patio structures for two yurts at Mosquito Lake Campground, including a like-for-like replacement of decking, railing, awning/cover, spindles, and lattice. The scope of work specifies commercial-grade composite decking in a brown shade, with railing, lattice, awning/cover, and spindles potentially constructed from stock vinyl or aluminum materials. The project encompasses approximately 36 feet by 23 feet yurt dimensions and includes an ADA-accessible walkway ramp and patio awning/deck cover. Work may commence on November 1st, 2025, during daylight hours and must be completed by December 31st, 2025. Prior to submitting a bid response, offerors are strongly encouraged to conduct a site visit with Park Manager Salim Fadzl at (234) 430-6039 to review facility specifications and project requirements. The contract will be awarded to the lowest responsive and responsible bidder based on a total lot calculation methodology. The contract is valid through June 30th, 2026.

Pricing must include all shipping costs, and bidders must submit comprehensive specifications for composite color selection and alternative materials for review and acceptance. The contract will be funded through the Ohio Department of Natural Resources with no State of Ohio sales tax applied. Invoices must be submitted directly to Ohio Shared Services via email at invoices@ohio.gov, fax at 1-614-485-1039, or mail to the designated Columbus address, and must include the assigned Purchase Order number, Origin-DNR-N13, along with itemized services, materials, labor charges, and a unique invoice number. Failure to bid all items may result in the bidder being deemed non-responsive, and failure to request a site visit prior to bidding will be insufficient reason to support contract release requests. The awarded supplier may be required to sign an Addendum to Bid Quote or Personal Services Agreement, and all work must include dismantling existing deck structures, hauling away debris, and securing the work area throughout project completion.
