About this file
This document is a Request for Quote (RFQ) issued by the Illinois Department of Veterans Affairs for five (5) battery-operated shower chairs. The chairs must meet specific requirements including an adjustable headrest, tilting adjustment, swinging armrests, upholstered seat with hygiene opening, four (4) braked 5" swiveling castors, height adjustable and removable footrests, and a 330lb weight capacity. The vendor must provide a total quote amount, estimated delivery time, and agree to warranty all products for a minimum of one (1) year. Vendors must submit their quotes within 180 days, and the agency reserves the right to reject any or all offers. The winning vendor will be selected based on the lowest total bid.
The RFQ specifies that all quoted prices must include shipping and handling, and no price increases will be accepted. However, in the event of a price decrease, the agency is guaranteed to receive the lowest price. The vendor must also agree to partial shipment of the equipment in case of a delay in the total order. This RFQ does not mention any set-asides, incumbent vendors, or specific funding sources and requirements.
