About this file

This is a Request for Quote (RFQ) template issued by the Illinois Department of Public Health (IDPH) for Informal Dispute Resolution (IDR) services. The RFQ seeks qualified vendors to provide written and telephonic IDR services on a fee-for-service basis for Skilled Nursing Facilities, Nursing Facilities, and Intermediate Care Facilities for Individuals with Intellectual Disabilities. This is a two-year contract, with pricing to be submitted on an hourly rate basis. The RFQ template establishes the framework for vendors to submit quotes and outlines the requirements and evaluation criteria that will be used in the selection process.

The RFQ requires vendors to submit hourly rate quotes for the provision of IDR services during the two-year contract period. Additional details regarding specific service delivery requirements, performance standards, staffing qualifications, and evaluation methodologies are contained within the full RFQ documentation. Prospective vendors should review the complete RFQ for comprehensive information on scope of work, deliverables, and all submission requirements necessary to prepare a competitive quote.
