This is a combined synopsis/solicitation for commercial products or commercial services prepared in accordance with the format in subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation;

proposals are being requested and a written solicitation will not be issued. The solicitation is being issued using Simplified Acquisition Procedures.

The competition is set-aside for small businesses. The North American Industry Classification System Code (NAICS) for this requirement is 562112, Hazardous Waste Collection with a size standard of $41.5 million.

This solicitation incorporates the provisions and clauses in effect in Federal Acquisition Circular 2022-05 dated March 7, 2022 and Defense Publication Notice 20220318 dated March 18, 2022.

Requirement Description

The Uniformed Services University of the Health Sciences (USU) has a requirement for a Firm- Fixed Price (FPP) five (5) year Blanket Purchase Agreement (BPA) for USU hazardous waste disposal. This requirement is intended to fulfill USU’s need for ad-hoc hazardous waste disposal services. Refer to Attachment 1: Statement of Work for the service requirements.

General Information

Basis of Award - Lowest Price Technically Acceptable (LPTA)

The Government intends to award a firm-fixed price Blanket purchase agreement (BPA) to the responsible, responsive vendor(s) whose quotes meet all of the requirements of the Statement of Work (Attachment #1) and proposes the lowest total prices.

To be eligible for award, the vendors must provide a solution that meets all the requirements of Attachment #1 Statement of Work. Failure to quote on requirements outlined will result in rejection of the quote (non-responsive).

The Government reserves the right award one, multiple, or no BPA’s as result of this solicitation.

The Government reserves the right to award without discussions.

Questions

Offerors shall submit all questions about this solicitation in writing to the Contracting Officer and the Contract Specialist no later than the date and time specified above. All responses to the questions will be in writing, without identification of the questioner, and will be included in an amendment to the solicitation. It will be the responsibility of the Offeror to monitor SAM.gov for the issuance of any amendments to the solicitation.

Quote Submission

Offerors shall submit their quotes via email to the Contracting Officer and the Contract Specialist no later than the date and time specified above. FAX quotations will not be accepted. Quotes received after the response date and time will not be considered. Please reference the RFQ name and number in the subject line of email communications.

Cancellation

USU has the unilateral right to cancel service with a 24-hour notice to the awardee. If cancellation incurs a cancellation fee, the fee must be disclosed in the quote submission.

System for Award Management (SAM)

In order to be eligible for award, vendors must be registered and active in SAM (www.SAM.gov).

Instructions to Offerors

Technical Quote

1. Offerors must provide a detailed overview of services provided and technical solution to augment their quote.

2. Quote submissions must include the following information:

a. Company’s full name and address

b. CAGE, Unique Entity Identifier (UEI), and Tax Identification Number

c. Name, title, phone number, and email of the person authorized to sign contracts for the company

Price Quote

1. The quote should mirror (identify) the product lines in the Price Quote Table (Attachment 3). The Offeror may add lines to their quote if needed to account for additional items or for items that are separately priced.

2. The Offeror must provide a price for each item listed in the table and provide a total delivered price.

3. If an item is not separately priced, offerors may enter NSP (not separately priced) or $0.00 in the price column. For example, if shipping is included in the price, enter NSP or $0.00 in the price column.

4. Quote prices shall be valid for a minimum of 90 days.

Provisions and Clauses

The solicitation and contract are subject to the FAR provisions and are also subject to the Defense Federal Acquisition Regulations (DFARS) provisions and clauses listed in Attachment #4.
