Description of Services
The U.S. General Services Administration (GSA), Great Lakes Region, Region 5, Fire Protection and Safety
Branch requires that facility assessments for Safety and Health are required to be conducted. We are requesting that two facility surveys be conducted: an Annual OSH Survey and if applicable, the Safety and Health (S&H)
Risk Management Survey (RMS) templates.
The Annual OSH Survey and the identified Safety and Health (S&H) Risk Management Surveys (RMS), when conducted, will be completed using GSA’s Safety and Health National Risk Management Survey (RMS)
Templates (separate templates for OSH and S&H RMS).
The Great Lakes Regional (R5) Fire Protection and Safety Branch is responsible for ensuring all Federally
Owned and Leased buildings are provided with an adequate level of safety. The Fire Protection and Safety
Branch serves as the authority having jurisdiction for GSA and issues Certificates of Occupancy for all new construction, renovations, and occupancy changes in federally owned buildings.
A. The Annual OSH Survey and the Safety and Health Risk Management Survey (RMS) Templates are a quantitative method to collect consistent and comparable facility data for compliance with safety regulations. The intent of the surveys is to assess the building, its systems, along with the operations & maintenance (O&M) and janitorial operations.
B. The purpose of conducting the Safety and Health RMS Survey Templates is to ensure that the federal government is not exposing the public, contract employees and GSA employees to any safety hazards, and that non-government GSA contracted building operation employees are operating within established
29 CFR 1910 OSHA regulations as well as 29 CFR 1960 where applicable. Conducting the Safety and
Health RMS requires an inspector or inspection team to walk and inspect buildings, building systems, and observe and interview GSA contracted maintenance and operational (O&M) and janitorial employees. The purpose is to determine if there are any safety related hazards, and what is the risk of injury, loss of life, property, or interruption to the Federal Tenant Mission, regarding compliance with safety regulations and industry related consensus standards. The surveys will identify safety compliance findings, and an evaluation of the level of risk and recommendations that need to be taken by GSA for the building or its operations & maintenance (O&M) and janitorial contractors to achieve compliance with safety standards.
The scope of this contract involves all pertinent conditions and work tasks performed by GSA employees or
GSA contractor activities that may affect GSA employees in accordance with requirements of the Occupational
Safety and Health Act of 1970, 29 CFR 1910; 29 CFR 1960, "Basic Program elements for Federal Employees
OSH Programs and Related Matters"; Executive Order 12196; the GSA Handbook ADM 5940.2 (March 19, 2019); and applicable consensus standards.
Place of Performance

Owned Locations, thirty five (35) that are Annual OSH Surveys, and ten (10) are combined Safety and Health
Risk Management Surveys (RMS) and Annual OSH Surveys. OSH and RMS templates will be provided by

B. PRE-SITE PREPARATION:
1) GSA will provide a list of all Property Managers, Operation Managers and/or Building Service Specialist with telephone numbers to the contractor upon Award and/or after a successful Kick-Off meeting. GSA will provide the most recent OSH and RMS survey templates (at the time of award/NTP) for all the proposed surveyed locations to the contractor and any past completed surveys (if conducted and relatable). The FY25 OSH and S&H RMS survey templates must be used / populated by the contractor.
A sample OSH and S&H RMS survey Excel template will be provided as part of the solicitation to represent the typical survey template format layout to be used for all Federally Owned and/or Leased properties under this contract.
2) Contractor will prepare and submit a site visit notification email to the GSA COR prior to conducting an inspection detailing the purpose of the inspection, what to expect, and contacts they will need to speak with. A blank RMS survey sample template for each property will be attached within the email so the building manager will know what they can be prepared for in advance. The COR will forward this email to the Building Manager. Within 2 days of the email being sent, the contractor may contact the Building Manager to make an appointment to gain access into the building to survey all areas of the building that are necessary, including the roof and the exterior of the building. The contractor will reach out to the COR if, after reaching out to the Building Manager, no response or return call needed to schedule the inspection has been received within 3 business days of the initial call.
Building managers are the GSA facility staff responsible for contacting contract maintenance and janitorial staff to be available during the Risk Management Survey (RMS). The maintenance and janitorial contractors have been awarded the contract to conduct the maintenance and janitorial work needed to operate the building and have been hired by GSA. The maintenance and janitorial contractors may be separate companies or the same organization, depending on the facility contract(s). Conducting the survey will include inspecting contractor workspace, equipment used, records, safety plans, PPE, tools, chemicals and how they are stored, handled.
3) A post award kick-off meeting will be scheduled immediately following award (telephonically or in person) with the GSA Property Management Team (or their representatives). Site coordination for dates/times of surveys will be determined after the Notice to Proceed (N.T.P.) has been issued by the
GSA Contracting Officer. Contractor shall record all correspondence. Contractor shall send an email confirming the date and time of the survey to the Building Manager / representative contacts, the
COR, and to the Fire Protection and Safety Branch representative Matthew T. Smith at matthew.smith@gsa.gov.
4) The kick-off meeting shall discuss the necessary topics and background including, but not limited to:
a) The schedule/ “milestones” and plans for conducting the surveys.
b) The GSA office location for the annual OSH survey.
about:blank
c) Secured areas in the building and access to those.
d) Types of mechanical systems and operations conducted in the building.
e) Preventive maintenance records, Inspection, and required Safety related plans and programs.
f) Navigation of the Excel spreadsheet document(s).
g) Additional items to be request before a site visit can be found in this document listed under
Paragraph A.), Section 5), Item h., items a. thru r., shown above.
C. ON-SITE PROCEDURES:
1) Upon every arrival at the facility to be surveyed, the contractor shall meet with a GSA Property Manager.
The survey shall not begin until this meeting is accomplished and documented.
2) A GSA Property Manager shall arrange for access to all spaces and documentation (electronic and/or paper) that the contractor needs to view.
3) The contractor shall conduct a complete and thorough walk-through of the related building areas and records review. The contractor is not required to walk-through all Tenant spaces as the intent is to access
Mechanical rooms, Equipment rooms, O&M and Janitorial spaces / offices, etc., to provide survey capabilities to document findings within the Excel survey template requested items. The contractor will record all conditions affecting risk, both positive and negative as deemed necessary. In addition, the contractor shall verify the status of any projects contained in the information provided by the COR. All spaces, except for tenant occupied spaces, in Government-owned buildings, must be surveyed.
4) The contractor must work with the GSA Property Management team to coordinate time for any questions they cannot answer with the O&M and Janitorial contractors to complete the S&H RMS survey templates.
D. CLOSING CONFERENCE:
At the conclusion of the building walk-through, the Surveyor must have a virtual teleconference or in-person meeting with the survey point of contact to discuss the conditions of risk observed during the walk-through. If any findings are deemed by the contractor to be scored as a RAC-1 or a RAC-2, those findings must be brought up immediately to the GSA Property Manager and the C.O.R. The contractor must also attend a virtual meeting to include the GSA Occupational Safety and Health Manager, the GSA Project Manager/C.O.R. (Matthew
Smith), as well as the GSA Property Mangers to review the findings within the RMS Survey Templates. The purpose is to discuss the survey observations. The contractor can’t have a closing conference with a contract employee (i.e.; O&M contractor). The contractor must review all items that were found to be deficient and review abatement recommendations as they relate to the Risk Assessment Condition (RAC) scoring.
E. SURVEY REPORTS:
1) File Format: The contractor must not rename the shared Excel spreadsheet documents in any manner and these documents must be return to the GSA with the originally shared File format / naming structure as to assure compatibility with GSA electronic systems.
2) General: The Building Information tab is pre-populated (any areas not populated will need to be populated by the contractor once they have gathered that information from the Property Manager). These documents contain Sensitive But Unclassified(SBU) Information.
3) If a Risk Management Survey (RMS) tab is not applicable to the property, then the contractor must provide a RAC score of 6 in the “User Adjusted RAC” field, and must provide a reason within the user adjusted RAC score in the “Explanation of User Adjusted RAC” field, in order to achieve a total RAC score for that building, which is calculated and displayed in the “RAC Summary” tab.
4) Items that require no action to be taken within these RMS surveys must be scored as a RAC 6, even when observational comments are provided. Items which have actions that must be taken, must only occur if the
RAC scores are RAC 1 thru 5.
5) Notification of the completed RMS Survey template Excel spreadsheet documents, with a completed
RAC summary score, must be sent via email, using the contractors newly established Government G-mail email address to the COR and Regional Fire Protection and Safety Branch representative, Matthew T.
Smith at matthew.smith@gsa.gov.
6) Each completed survey shall be included in the shared Excel spreadsheet document. The shared Excel spreadsheet document for each of the listed properties will be provided by the GSA. The information required to be entered on the spreadsheet is provided within the Excel spreadsheets documents question fields. The shared Excel spreadsheet document will be shared post-award using the contractor’s GACA compliant shared Google Drive folder(s).
7) The GSA’s assigned Occupational Health and Safety Manager will have 10 working days to perform a review of the contractors completed shared Excel spreadsheet draft survey documents and for all comments to be incorporated into the Excel spreadsheet document by the contractor before contract completion/sharing with Property Managers and being uploaded into the IRIS FMA module within
Central Office.
8) Once the Safety and Health Risk Management Survey template is completed and provided to the GSA project manager / C.O.R. (Matthew Smith) from the contractor, that document will be uploaded, not finalized, into the Central Offices IRIS FMA Module for confirmation of the contractors submitted Excel document in that it has not been altered and can be successfully uploaded into the referenced system electronically. If the contractor provided Excel survey template cannot be successfully uploaded into the
IRIS FMA Module, the contractor must revise, and resubmit the Excel survey template in a usable Excel format.
NOTE:
The information below has been provided to further detail the Risk Assessment Codes (RAC) in addition to the guidance already provided within the Risk Management Survey templates tab titled "RAC Guidance".
Assign Risk Assessment Codes (RAC)
Evaluate the condition severity (Table 1) and determine the incident probability (Table 2) if the incident did occur. Then, using the RAC Matrix (Table 3), determine the Risk Assessment Code number for each identified risk condition.
The level of risk for each analyzed condition is expressed in terms of a Risk Assessment Code (RAC). The
Risk Assessment Code(s) quantifies identified hazardous conditions and allows them to be prioritized. The
RAC number reflects the risk potential of the resulting incident and not the individual event(s) contributing to the incident.
The following steps indicate how RAC values are assigned to potential conditions and their associated undesirable events:
Develop scenarios that describe how the most reasonable undesirable events affect the identified potential conditions.
Estimate the severity of each condition. The condition severity is based upon the potential impact of the condition if the undesirable event(s) occur. Quantitative risk assessment techniques, computer modeling, published information and professional judgment are to be used to determine the severity. See Table 1, Condition Severity.
