About this file
This document is a Request for Quote (RFQ) issued by the Illinois Department of Human Services (DHS) for reusable cloth face masks. The RFQ is seeking a variety of mask styles, including 8,000 ear loop masks, 8,000 ear loop masks with adjustable ear loops, 8,000 masks with ties, 1,000 gaiter-style masks, and 5,000 mask strap adjustable extender/ear hook strap buckles. The masks must be at least 2 layers, with the outer layer made of tightly woven cotton and the inner layer being comfortable for continuous use for at least 4 hours. The RFQ includes specific dimensional requirements for the masks. Vendors can bid on a single item or multiple items, and contracts will be awarded by line item. Responses must be submitted by July 15, 2020 at 10 AM via BidBuy or email. The winning vendor will be chosen based on the lowest total bid for all products. DHS may require bidders to submit evidence of their qualifications, and the agency reserves the right to reject any offers.
The RFQ includes pricing requirements, stating that all quoted prices must be valid for 90 days from the date of submission, and no price increases will be accepted. In the event of a price decrease, the agency is guaranteed to receive the lowest price. The RFQ also notes that DHS may require bidder(s) to submit evidence of their qualifications at such times and under conditions, as it may deem necessary.
