(IAW FAR 12.603)
Solicitation Number: FA8126-23-Q-0003
DISA CSSP – MISSION PARTNER
HFGCS – SENSOR REPLACEMENT
This is a combined synopsis/solicitation for commercial items prepared in accordance with the formats in Subpart 12.6 and Subpart 13.5, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2023-04.
This solicitation is being issued as Request for Quote (RFQ) IAW FAR Part 13.
The Air Force Life Cycle Management Center (AFLCMC/HBK) at Tinker Air Force Base Oklahoma, intends to award a Competitive, Small Business Set-Aside, Brand Name, Firm-Fixed-Price contract for the acquisition of material listed below.
This solicitation is a brand name only requirement. Substitute/Equal Items are not permitted for this solicitation. Quotes that include substitute/equal items will not be considered for award. The contract will be issued for a Sensor Replacement. All responsible sources may submit a capability statement and proposal which shall be considered by the agency. Please see attached Statement of Work for details.

Vendor is required to “wet” sign/date and fill in the vendor information, delivery and pricing on page 3, complete the CLIN Unit Pricing and Amount on pages 4-8, the Tax Clause, 52.209-11, on page 38, the clause 52.204-26 on page 34, and if needed the clause 52.204-24 on page 36. The Vendor shall complete only paragraph (b) of 52.212-3 if there have been changes to the vendor’s current SAM registration. If the Vendor has not completed the annual representations and certifications electronically, the Vendor shall complete paragraphs (c) through (u) of this provision. If 52.212-3 is not returned with the quote, the Vendor is affirming that the current SAM registration is current, accurate, and complete.
Submit proof of technical evaluation criteria needed to fulfill this requirement. Please return 1 signed copy to the issuing office. Contractor agrees to provide all products set forth or otherwise identified above and on any additional sheets subject to the terms and conditions specified herein.
VENDOR SHOULD PROVIDE A COMPANY QUOTATION SHEET IN CONJUNCTION WITH THIS RESPONSE PAGE. The quotation sheet must provide adequate documentation showing that the item being quoted meets the specifications listed in the Statement of Work and Item Description in order to be considered for award.

EVALUATION OF RESPONSES:
The Government intends to award a single purchase order resulting from this solicitation to the responsible vendor who is technically acceptable with the lowest evaluated price. The Award will be conducted and evaluated under the provisions of FAR Part 12, Commercial Items and FAR Part 13, Simplified Acquisition Procedures.
The responsible vendors must be registered in the System for Award Management (SAM) database (available at:
https://sam.gov/content/home. Lack of registration in SAM will qualify the contractor as ineligible for award.

(d) Comptroller General Examination of Record. (a) Definitions. As used in this clause—
(1) “Contract financing payment” and “invoice payment” have the meanings given in section 32.001 of the Federal Acquisition Regulation.
(2) “Electronic form” means any automated system that transmits information electronically from the initiating system to all affected systems. Facsimile, e-mail, and scanned documents are not acceptable electronic forms for submission of payment requests. However, scanned documents are acceptable when they are part of a submission of a payment request made using Wide Area WorkFlow (WAWF) or another electronic form authorized by the Contracting Officer.
(3) “Payment request” means any request for contract financing payment or invoice payment submitted by the Contractor under this contract.
(b) Except as provided in paragraph (c) of this clause, the Contractor shall submit payment requests and receiving reports using WAWF, in one of the following electronic formats that WAWF accepts: Electronic Data Interchange, Secure File Transfer Protocol, or World Wide Web input. Information regarding WAWF is available on the Internet at https://wawf.eb.mil/ .
(c) The Contractor may submit a payment request and receiving report using other than WAWF only when—
(1) The Contracting Officer authorizes use of another electronic form. With such an authorization, the Contractor and the Contracting Officer shall agree to a plan, which shall include a timeline, specifying when the Contractor will transfer to WAWF;
(2) DoD is unable to receive a payment request or provide acceptance in electronic form;
(3) The Contracting Officer administering the contract for payment has determined, in writing, that electronic submission would be unduly burdensome to the Contractor. In such cases, the Contractor shall include a copy of the Contracting Officer’s determination with each request for payment; or
(4) DoD makes a payment for commercial transportation services provided under a Government rate tender or a contract for transportation services using a DoD-approved electronic third party payment system or other exempted vendor payment/invoicing system (e.g., PowerTrack, Transportation Financial Management System, and Cargo and Billing System).
(d) The Contractor shall submit any non-electronic payment requests using the method or methods specified in Section G of the contract.
(e) In addition to the requirements of this clause, the Contractor shall meet the requirements of the appropriate payment clauses in this contract when submitting payment requests.

(d) WAWF training. The Contractor should follow the training instructions of the WAWF Web-Based Training Course and use the Practice Training Site before submitting payment requests through WAWF. Both can be accessed by selecting the “Web Based Training” link on the WAWF home page at https://wawf.eb.mil/ .
