About this file
This document is a Request for Quotation (RFQ) issued by the Ohio Department of Natural Resources (DNR) for the East Fork State Park Bethel Concord Trail Project, specifically for repairs to an existing drainage culvert. The project requires the delivery of 75 yards of QC MS concrete to the end of Bethel Concord Road in Bethel, Ohio. Contractors will need to drive concrete trucks down a dead-end road, pour concrete into forms, and have the option to turn around or back out. Site visits are available by appointment only, and interested bidders must contact Bryan Curless at (513) 205-7025 to schedule. The contract will be awarded to the lowest responsive and responsible bidder, with the evaluation based on the low lot total, which is calculated by multiplying unit costs by estimated usage.
The document outlines specific invoicing requirements for the contract, which is valid through June 30, 2025. Invoices must be submitted directly to Ohio Shared Services (OSS) via email, fax, or mail, and must include detailed information such as the Purchase Order number, supplier name, service facility details, invoice number, date of service, itemized services, and line charges. No Ohio sales tax is to be applied, and the remit-to address must match the records for payment. The contract emphasizes that failure to request a site visit will not be grounds for release from the contract, and any goods or services received after the June 30, 2025 deadline will require a new Purchase Order.
