About this file

This is an Addendum 2 to a Request for Qualifications (RFQ) issued by the Oregon Department of Forestry for GNA Calloway Heritage Surveys to be conducted in the Willamette National Forest. The addendum modifies the original solicitation documents by updating minimum requirements, key personnel qualifications, and deleting Attachment B (Reference Check Form). The document outlines detailed qualifications for archaeological personnel, including the Principal Investigator, Archaeological Crew Chief, and Archaeological Crew members, with specific requirements for education, field experience, archaeological survey expertise, and professional certifications.

The key modifications include removing the requirement to complete a Reference Check Form and adjusting the minimum qualifications for archaeological personnel. Offerors must submit a Statement of Qualifications that will be evaluated on a pass/fail basis, providing resumes for key personnel and any subcontractors. The qualifications are extensive, requiring graduate and undergraduate degrees in archaeology or related fields, specific field experience, professional certifications, and demonstrated competence in archaeological survey, excavation, and artifact identification. The addendum emphasizes the need for personnel to meet Secretary of the Interior's Professional Qualification Standards and Oregon Qualified Archaeologist requirements.
