About this file
This document is a Past Performance Information Worksheet (PPIS) template for a federal contract solicitation, designed to capture detailed historical contract performance information from potential offerors. The form requires comprehensive details about previous contracts, including contract specifics (agency, number, type, performance period, contract value), customer points of contact, technical approach, key personnel, and relevance to the current solicitation.
The template is specifically structured to support an evaluation of past performance for an Underground Storage Tank Inspection requirement at Scott Air Force Base, issued by the 375 Contracting Squadron. Offerors must complete the form with precise information about their previous contracts, highlighting technical capabilities, staffing approaches, and compliance with small business utilization requirements. The worksheet is intended to help the Department of the Air Force's Air Mobility Command assess a contractor's qualifications and past performance in preparation for awarding a firm-fixed price and time-and-materials purchase order for underground storage tank inspections.
