About this file
This is a Past Performance Information Sheet template for offerors responding to a Request for Quotation (RFQ) for vehicle barrier systems maintenance, repair, and installation services. Offerors must submit three past performance references that are recent (within five years of RFQ issuance) and relevant to work involving maintenance, repair, and installation of vehicle barrier systems. The form requires detailed project information including the project title, contract/order number, performing company designation (prime, subcontractor, or teaming partner), customer/agency details with primary point of contact information, contract value, period of performance dates, and a comprehensive narrative describing work performed with specific attention to how the project relates to the RFQ's Performance Work Statement scope, magnitude, and complexity.
The template also requires offerors to provide evidence of performance quality through CPARS ratings (if available), letters of recommendation, or other performance assessments such as customer surveys. Offerors must indicate whether the listed customer point of contact is willing to participate in a performance interview or survey. By submitting the form, offerors grant the U.S. Government permission to contact references to verify information and assess performance on the cited contracts. The form includes specific sections for documenting Quality, Schedule, Cost Control, and Management ratings from CPARS records where applicable.
