About this file

This document is a Request for Quote (RFQ) issued by the Illinois Department of Veterans Affairs for the purchase of insulin syringes over a two-year period from July 1, 2018 to June 30, 2020. The agency is seeking to purchase 2,000,003,005 safety insulin syringes with automatic needle retraction, with the quantities split evenly between fiscal years 2019 and 2020. Vendors are required to provide per-case pricing and manufacturer details, and the award will be made to the bidder with the lowest total bid. The RFQ outlines that actual quantities ordered may vary based on the facility's needs, and partial shipments are acceptable in the event of delays.

The pricing terms stipulate that all quoted prices must be valid for 180 days and include shipping and handling charges, with no price increases allowed. The agency reserves the right to reject any or all offers. Vendors must agree to provide a minimum 1-year warranty on all products.
