ABSTRACT

General Instructions:

The Offeror should review submission instructions in accordance with FAR 52.212-1 Instructions to Offerors-Commercial Items (Nov 2021) that have been incorporated by reference as well as RFQ submission instructions that start on page 14 of this document. The following is general information provided in addition to the RFQ instructions that can be found starting on page 14 of this document.

1. The Offeror shall provide a schedule of prices that will include individual pricing for all analyses required under the page 4.

2. The Offeror shall complete Offeror Representations and Certifications.

3. The Offerors shall provide company information to include company name, address, Sam Unique ID, main point of contact, and the companies’ commercial terms and conditions.

4. Provide all information above as well as any other information required under the RFQ submission instructions that start on page 14 to the following individuals:

3. Addendum to FAR 52.212-4 — Additional Clauses

Post-Award Meeting

The contractor shall be required to participate in 1) a post-award kick-off meeting. The kick-off meeting shall be conducted by a conference call with the GSA Contracting Officer’s

Representative and any other GSA personnel identified, for discussion and interpretation of all instructions.

Place and Period of Performance

The contractor shall commence the work upon Notice to Proceed (NTP). The Period of

Performance (POP) shall be (365) days or one year.

(d) Comptroller General Examination of Record. The Contractor shall comply with the provisions of this paragraph (d) if this contract was awarded using other than sealed bid, is in excess of the simplified acquisition threshold, as defined in FAR 2.101, on the date of award of this contract, and does not contain the clause at 52.215-2, Audit and Records-Negotiation.

(1) The Comptroller General of the United States, or an authorized representative of the

Comptroller General, shall have access to and right to examine any of the Contractor’s directly pertinent records involving transactions related to this contract.

(2) The Contractor shall make available at its offices at all reasonable times the records, materials, and other evidence for examination, audit, or reproduction, until 3 years after final payment under this contract or for any shorter period specified in FAR subpart 4.7, Contractor

Records Retention, of the other clauses of this contract. If this contract is completely or partially terminated, the records relating to the work terminated shall be made available for 3 years after any resulting final termination settlement. Records relating to appeals under the disputes clause or to litigation or the settlement of claims arising under or relating to this contract shall be made available until such appeals, litigation, or claims are finally resolved.

(3) As used in this clause, records include books, documents, accounting procedures and practices, and other data, regardless of type and regardless of form. This does not require the

Contractor to create or maintain any record that the Contractor does not maintain in the ordinary course of business or pursuant to a provision of law.

(e)

(1) Notwithstanding the requirements of the clauses in paragraphs (a), (b), (c), and (d) of this clause, the Contractor is not required to flow down any FAR clause, other than those in this paragraph (e)(1) in a subcontract for commercial products or commercial services. Unless otherwise indicated below, the extent of the flow down shall be as required by the clause-

0.2 Submission Formatting

Submissions shall be in the following format:

(1) Pages shall be 8½x11-inch white paper, ½-inch margins on the top and bottom of the pages and 1-inch margins on the left and right sides of the pages, with lettered/numbered dividers for each section to correspond with the tabs above. Except—

(a) When appropriate, pages may be 11x17 inches and with the same margins but will be counted as 2 pages towards any applicable page limitation defined above.

(b) Resumes and financial statements may have margins and font sizes different from those listed above but shall be legible and shall be submitted on 8½x11-inch paper.

(2) All text shall be in either Arial or Times New Roman font at an 11-point minimum.

(3) The information provided in the submission shall be easily reproducible by normal black and white photocopying machines.

(4) Materials submitted become the property of the Government and shall not be returned.

(5) Spreadsheets shall be provided in Microsoft Excel native format and include any formulas used so they may be verified; otherwise, submissions shall be provided in Word or PDF format. No other formats will be accepted.

1. VOLUME 1: Representations, Certifications, and Other Statements of Offerors

1.1 Offeror Representations and Certifications

(a) Offerors shall submit with their proposal the attached Respondent’s Representations and

Certifications (“Attachment 2 –Offeror’s Representations and Certifications”).

(b) Offerors are reminded their SAM registration status must be “Active”, at the time their offer is submitted, to be considered for award (see 52.204-7). Offerors submitting a proposal in response to this Solicitation shall complete electronic Annual Representations and

Certifications in conjunction with required Entity registration in System for Award

Management (SAM), accessed via System for Award Management.

2. VOLUME 2: Technical Proposal

The Technical proposal shall include the information requested below for the non-price factors described in the Evaluation Criteria.

Offeror must provide examples of past experience with references for past performance.

Volume 3: Price Proposal

The Offeror shall provide a price proposal that provides pricing for all lab analysis in the statement of work.

3.1 FAR 52.212-2 EVALUATION—COMMERCIAL PRODUCTS AND COMMERCIAL SERVICES

(NOV 2021)

The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the

Government, price and other factors considered. The offeror who presents the best value to the Government will be awarded the contract. The following factors shall be used to evaluate offers:

FACTOR 2: Past Performance

The evaluation factors listed above, when combined, are equal when compared to price.

(b) Options. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced.

Evaluation of options shall not obligate the Government to exercise the option(s).

(c) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer’s specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award.
