About this file
This is a Request for Quotation (RFQ) issued by the Southern Arizona VA Healthcare System for the procurement of radiofrequency ablation (RFA) generator equipment. The requirement is for one RFA Generator RFG-X1-120V GX1 Radiofrequency Gen 120V Kit or equivalent, along with one Generator Output Test Plug (RFG-TP) and one 25-pack of GX1 Split Wired Grounding Pads (1285H-25). The RFA generator is a critical medical device used in pain intervention procedures to reduce chronic pain and opioid use, with procurement driven by high utilization of the single existing unit and resulting extended patient wait times. The quoted equipment must be new, unused, FDA-cleared Class II medical device compliant with applicable safety standards, featuring four independent output channels, capacitive touchscreen with auto-calibration, Contact Quality Monitoring, durable aluminum chassis, and USB support for software updates. The procurement is structured as a 100% Total Small Business Set-Aside under NAICS code 339113 (Surgical Appliance and Supplies Manufacturing) with a 750-employee size standard, requiring offerors to certify small business status and provide UEI and CAGE codes. Additionally, all products must comply with Buy American Act requirements with country-of-origin certification.
Quotations must be submitted electronically to sabrina.schofield@va.gov by July 31, 2026, at 12:00 PM PST/3:00 PM ET, with all questions due by July 29, 2026. Delivery is required within 60 calendar days of order receipt to the Southern Arizona VA Healthcare System, with installation and training not required. Proposals must include detailed technical specifications confirming all salient characteristics, product datasheets, itemized pricing, delivery timeline, warranty details, and business information. Evaluation will follow a Best Value Trade-Off process prioritizing technical capability, past performance on federal or VA contracts, delivery and installation ability, support and warranty scope, and price. Standard manufacturer warranty is sufficient with no extended warranty required. The government is not obligated to award a contract or reimburse proposal preparation costs.

Request for Quotation (RFQ)
1. Contract Title RFA Generator RFG-X1-120V GX1 Radiofrequency Gen 120V Kit or Equivalent
2. Background The RFA generator is a critical device used during radiofrequency ablation pain intervention procedures to reduce chronic pain and decrease opioid use in patients. Currently, only one RFA Generator is available daily in the Desert Willow Pain Procedure room at the Southern Arizona VA Healthcare System. The high utilization limits the number of procedures that can be performed, resulting in extended wait times and suboptimal patient care. An additional RFA generator would allow for increased procedural capacity, especially for procedures conducted in the OR, resulting in improved care continuity and reduced reliance on community care. The RFA generator is essential, as it enables targeted pain relief by disrupting pain-signaling nerves.
3. Scope This procurement is for the purchase of an RFA generator only. The equipment will support a wide range of interventional pain procedures, including but not limited to knee, hip, sacroiliac, and facet joint interventions.
4. Salient Characteristics (Minimum Requirements) The quoted product must meet or exceed the following:
• Four independent output channels (for treatment of multiple sites and operation in monopolar, bipolar, or pulsed RF modes)
• Easy-to-use capacitive touchscreen with auto calibration
• Contact Quality Monitoring (CQM) for accurate electrode and ground pad connection
• Durable aluminum chassis and standard ground pad compatibility
• USB support for software updates and system upgrades
• Designed for multiple interventional pain procedures (e.g., facet, sacroiliac, and peripheral nerve ablations)
• FDA-cleared, Class II medical device; compliant with applicable safety/regulatory standards
• Standard voltage configurations (120V/220V/240V) for clinic compatibility
• New equipment only; used or refurbished items are not acceptable
