STATEMENT OF WORK (SOW)

1. Contract Title.

Medical Vacuum Pump

2. Background.

The Spinal Cord Injury (SCI) Unit at the New Mexico VA requires a medical vacuum pump for patient care. The previous dental vacuum pumps are at the end of their serviceable life expectancy. To maintain compliance and ensure continuity of care and patient safety, the facility must have a primary and backup medical vacuum pump system with dedicated piping installed as soon as possible. The permanent VA-owned medical vacuum pump will be provided and installed by the awarded vendor as per the manufacturer and NFPA 99 (National Fire Protection Association) specifications in congruence with guidance from the appropriate VA staff.

3. Scope.

· Provide and install one permanent medical vacuum pump system capable of handling all demands of the Spinal Cord Injury unit

· Provide 3rd party verification before system is put into operation

· Provide services and maintenance as guaranteed by the warranty and vendor contract

· Assist in transitioning from the current rental system to the permanent system to ensure a smooth transfer

· Remove rental system after the new quadplex system is installed and put into service

· Installation of the line voltage required by the new medical vacuum pump source

· Installation of all NFPA compliant medical vacuum pump alarm signals from the new source to the new MAP (Master Alarm Panel) location at the Building 45 Nurse Station

4. Specific Tasks.

· Contractor will deliver medical vacuum pump and accessories to the New Mexico VA Health Care System (NMVAHCS) and install

· System will be verified before putting into use

· All service and maintenance required on the vacuum pump will be provided by vendor to include response in emergency failure of the vacuum system

5. Performance Monitoring.

Both the VA point-of-contact and the plumbing shop supervisor will confirm delivery and installation of the equipment and that all services and maintenance have been rendered.

6. Government-Furnished Equipment (GFE)/Government-Furnished Information (GFI).

None

7. Risk Control.

None

8. Place of Performance.

Medical vacuum pump and accessories will be delivered to the back loading dock of (Bldg. 41) NMVAHCS, 1501 San Pedro Dr. SE, Albuquerque, NM 87108.
