B.4 STATEMENT OF WORK

Title:

Software License Renewal Joint Commission Resources Accreditation Manager Plus (AMP) and Tracer Program

Objective:

Implementation and yearly renewal of the multiple Licenses of the Tracer Program throughout Hospitals and Medical Centers in VISN 20 using a base plus 4-year option year contract. The contract will include, but not limited to, licenses for Ambulatory, Behavioral Health and Home Care Tracers, per Price/Cost Schedule.

Purpose:

The AMP and Tracer programs are proprietary software programs developed and owned by the vendor Joint Commission Resources (JCR). These programs will be utilized by the Hospitals and Medical Center throughout VISN 20 for the purpose of continued survey readiness.

Background:

The VHA Office of Quality Management has a national contract for the Hospital Tracers with the Joint Commission Resources Review already based upon the AMP software. Tracer license purchases require annual license renewal; therefore, the Tracer program is being consolidated and expanded throughout VISN 20. Due to the quantity of services purchased by the VHA, JCR offers a discount on add-on packages, including additional AMP and Tracer programs.

A key part of continuous processes improvement is the tracer methodology uses. The method allows for an organization to follow the experience of care, treatment, or services for patients through the organization’s entire health care delivery process. Tracers allow users to identify performance issues in one or more steps of the process or interfaces between processes. The Tracer tools and library provides pre-populated questions and is continuously updated.

Questions are designed, customized, and disseminated from the system level to each site.

Tracers with AMP is a software tool that allows to analyze organization's performance, prioritize improvements, assess organizational risk, increase collaboration and build automated reports for leadership and frontline staff. There are 15,000 pre-built, customizable tracers plus new and updated tracer templates added continuously throughout the year.

The Federal Information Security Management Act (FISMA) Federal Risk and Authorization Management Program (FedRAMP) is required for the Authority to Operate (ATO) the ‘Tracers with AMP’ e-product procured through the Applicable Publications requirement of the VHA Healthcare Accreditation Services.

Scope & Methodology:

The AMP and Tracer programs are utilized in services throughout VISN 20 as a means to ensure continued survey readiness. AMP software provides 100% of the Joint Commission standards for each of the Joint Commission Manuals in an electronic format. This program also allows the facility to evaluate how it is performing in each of the standards on an ongoing basis through the “scoring” program. This scoring is submitted to the Joint Commission annually during non-survey years as a component for the ongoing Joint Commission Accreditation. The Tracer programs allow for ongoing evaluation of key areas and areas of weakness as a method to monitor the effectiveness of education and interventions implemented. Tracers are also conducted post survey completion as a tracking mechanism to follow-up and demonstrate continued compliance in specific areas identified by the surveyors as areas in need of improvement.

The objective of the tracer tool is to identify risk points and safety concerns within different levels and types of care, treatment or services. Program-specific tracers focus on important issues relevant to the organization, such as clinical services offered and high-risk, high-volume patient populations.

Tasks & Deliverables:

The contractor shall provide access to Accreditation Manager Plus (AMP) and Tracers with AMP, developed by Joint Commission Resources, designed to anticipate, reduce, and report risks. The platform has a centralized repository for all quality and accreditation data, meeting digital compliance for security.

The contractor shall provide access to the JCR Software Portal upon contract award of the the Tracer programs and AMP. This access will be authorized for the duration of the license(s) annual Period of Performance with availability for renewal on an annual basis via option years awarded by the Contracting Officer.

The contractor shall be FedRAMP certified and shall provide Tracers with AMP software tool allowing VA to analyze facility performance, prioritize improvements, assess organizational risk, increase collaboration and build automated reports for leadership and frontline staff. The contractor shall grant access to the library of pre-built, customizable tracers plus new and updated tracer templates added continuously throughout the performance period(s). Tracers with AMP (Hospital) access and the FedRAMP Certification shall be granted / provided within 7 business days of Notice to Proceed.

The contractor will provide technical support as well as customer service support during VISN 20 business hours, Monday through Friday 8:00am to 4:30pm Pacific Time.

Updates to the software will be posted to the JCR Software Portal when indicated, with access to view throughout the duration of license.

Reporting Requirements:

Email notification of licensure expiration will be sent by JCR to individuals associated with the licensure issuance prior to expiration.

Reports are the backbone of effective communication in both academic and professional realms. Reports enhance ability to convey information and influence decisions, in the evaluation process. Report can describe evaluation of performance, progress, or outcomes. Reports act as a decision-making tool that can assist in making decisions based on facts and evidence. VA requires multiple reports from individual findings on surveys by facility to summation reports for annual findings and trends.

The objective is to provide efficient communication of all service activities, trends, findings associated with the surveys and related aspects for the VA to make well informed decision of potential strategic changes throughout the enterprise.

Period of Performance:

(B) The date of the first written demand for payment, including any demand for payment resulting from a default termination.

Subject to the terms of this contract, the Contractor shall be paid a percentage of the contract price reflecting the percentage of the work performed prior to the notice of termination, plus reasonable charges the Contractor can demonstrate to the satisfaction of the Government using its standard record keeping system, have resulted from the termination. The Contractor shall not be required to comply with the cost accounting standards or contract cost principles for this purpose. This paragraph does not give the Government any right to audit the Contractor's records. The Contractor shall not be paid for any work performed or costs incurred which reasonably could have been avoided.

(m) Termination for cause. The Government may terminate this contract, or any part hereof, for cause in the event of any default by the Contractor, or if the Contractor fails to comply with any contract terms and conditions, or fails to provide the Government, upon request, with adequate assurances of future performance. In the event of termination for cause, the Government shall not be liable to the Contractor for any amount for supplies or services not accepted, and the Contractor shall be liable to the Government for any and all rights and remedies provided by law. If it is determined that the Government improperly terminated this contract for default, such termination shall be deemed a termination for convenience.
