B.2 STATEMENT OF WORK
Window Cleaning Services
A. SCOPE OF WORK:
1. The contractor shall furnish all labor, material, equipment, supplies, transportation, and supervision to provide window washing services by means of a firm-fixed price contract, base plus four (4) option years, for the Central Texas Veterans Health Care System (CTVHCS), Olin E. Teague Veterans' Medical Center, 1901 Veterans Memorial Drive, Temple, TX 76504 (Bell County) and CTVHCS Doris Miller Department of Veteran Affairs Medical Center, 4800 Memorial Drive, Waco, TX 76711(McLennan County). This includes all equipment required by contractor to reach heights of all windows and aluminum screens. Razor blades or sharp scrapers shall not be used on the exterior windows that are made of lexan. Special care must be adhered to in the cleaning of Lexan glass; The contractor shall ensure that all work is to be performed in accordance with the guidelines established by Federal, State, and local ordinances.
2. Table A provides locations, building numbers, cleaning required, schedule of cleaning and number of floors per building to be cleaned. Table B provides photos of each building; however, the Contractor shall rely on their own data collection from site visits for proposal preparation. Any increased or decreased requirements may only be initiated or authorized by the Contracting Officer resulting. (See Table B for Photos)
3. Performance Period: Base plus 4 Option Years.

B. REQUIREMENTS:
1. The contractor shall quarterly clean the exterior windows, aluminum screens and exterior and interior entryways in Buildings 163, 204 and 202 at the Central Texas Veterans Healthcare System (CTVHCS) Olin E. Teague Veterans' Medical Center, 1901 Veterans Memorial Drive, Temple, TX 76504 (Bell County).
2. The contractor shall quarterly clean the exterior windows, aluminum screens and exterior and interior entryways in Buildings 11, 90, 91, 92, 93, 94, 202, 219, 226 at the CTVHCS Doris Miller Department of Veteran Affairs Medical Center, 4800 Memorial Drive, Waco, TX 76711 (McLennan County).
3. The contractor shall quarterly clean the exterior windows, aluminum screens and exterior and interior entryways in Buildings 2, 4, 7, 8 (First Floor only) at the CTVHCS Doris Miller Department of Veteran Affairs Medical Center, 4800 Memorial Drive, Waco, TX 76711(McLennan County).
4. During the base year only, the contractor shall remove all debris, cobwebs, and bugs from between the glass panels of double pane windows, clean glass surfaces and reseal double glass window unit in buildings 2, 11, 90, 91, and 92 at the CTVHCS Doris Miller Department of Veteran Affairs Medical Center, 4800 Memorial Drive, Waco, TX 76711(McLennan County).
5. Prior to start of work, all Contractor equipment shall be safety checked to conform to all applicable State Codes and OSHA regulations. The Government reserves the right to request the removal from the work site any supplies and/or equipment it deems does not meet the applicable State codes or OSHA regulations. In addition, it may request the halt of any unsafe practices observed in carrying out the contracted service. This will in no way relieve the Contractor of complying with the wash schedule.
6. The contractor shall wash all glass surfaces individually and thoroughly by hand in accordance with good commercial practices. Glass surfaces shall be left translucent and free from dirt, dust, streaks, tar, gum, squeegee, rag marks, water spots, paint, waterproofing compounds and/or other foreign substances.
7. The contractor shall inspect buildings 163, 204 and 202 at CTVHCS Olin E. Teague Veterans' Medical Center, 1901 Veterans Memorial Drive, Temple, TX 76504 (Bell County) and buildings 2, 4, 7, 8, 11, 90, 91, 92, 93, 94, 202, 219 and 226 at CTVHCS Doris Miller Department of Veteran Affairs Medical Center, 4800 Memorial Drive, Waco, TX 76711(McLennan County) to ensure that the buildings meet the structural requirements or need to enable safe climbing for window washing.
8. The contractor shall ensure that all windows are washed clean and free of streaks, smears, and visible soap residue. Accumulated dirt, paint specs, or other foreign debris must be scraped from windows. Frames shall be scrubbed to remove all dried dirt, insects, pigeon feces, debris, and other materials to be considered clean by building representative. Windowsills shall be washed clean, and all drippings wiped dry.
9. The contractor shall remove and clean all screens, where present, free from dirt, tar, gum paint, waterproofing compounds, and/or other foreign substances. They shall be replaced and firmly reattached. Opened windows and security screens shall not be left unattended for any reason!
10. All outside sills, casements, frames, dividers, tracks, and ledges (includes areas between screens and windows) shall be cleaned and left free from dirt, debris, tar, gum, waterproofing compounds, spider webs, wasps, bird nests, leaves, and/or other foreign substances.
11. All stains, paint, or putty discoloration’s which were applied to the windowpanes or caused by window cleaning shall be thoroughly removed from the panes, screens, sills, or other property.
12. All walls shall be protected to prevent damage and to eliminate safety hazards.
13. Contractor, while cleaning, shall inspect windows and surrounding area for voids, rotten wood, and other damage. Damage will be reported to the COR or designee at the close of each business day.
14. All tools, materials, equipment (scaffolds, ladders, scrapers, blades, rags, squeegees, steel wool, cleaning chemicals/compounds, etc.) shall be strictly controlled by the contractor for proper disposal and to prevent access by unauthorized personnel. This includes safeguarding the equipment and cleaning chemicals/supplies to prevent patients’ access and possible injury while the contractors’ employees are using the equipment.
15. Safety Data Sheets (SDS) for all chemicals, solutions, cleaning agents, etc., which will be used by the contractor shall be furnished to the designated COR prior to the starting of work by the contractor personnel.
16. Heavy steel hinged security screens are attached to interior wall windows on upper porches of patient occupied buildings. These screens are to be opened and cleaned by the contractor at the same time the window unit is being cleaned. VA personnel will assist, if necessary, in opening and closing of the security screens. Each security screen shall be firmly and properly secured after the cleaning is completed for each window.
17. Contractor personnel shall comply with Sub-Part D (walking-working surfaces) of OSHA General Industry Standards, Part 1910, and Title 29 of the Code of Federal Regulations.
18. If the COR does not accept windows and window units after the second cleaning, then the cost of that unit shall be deducted from the prorated original cost and work shall continue on other buildings/floors.
19. The contractor shall ensure that any chemicals used to clean the lexan glass will not cause any discoloration or other adverse reactions. Each windowpane shall be hand cleaned individually level with the window. Cleaning of upper story windows from ground level is not acceptable and will not be permitted!
20. Current OSHA Standards and Regulations will be strictly enforced. All equipment and supplies used in the performance of the contract shall be approved by the Environment Services Manager and Safety Officer prior to commencement of work.
21. The contractor shall furnish all equipment and supplies needed to carry out the window washing services specified at no extra cost to the Government. All equipment and supplies used must be capable of performing all operations in accordance with the Statement of Work.
C. CONTRACTOR PERSONNEL REQUIREMENTS:
1. The contractor shall provide experienced, qualified, and capable personnel to perform the work required by the contract.
2. Contractor’s personnel shall be fully knowledgeable of all safety and environmental requirements associated with the work they perform.
3. Contractor’s personnel shall speak, read, and comprehend the English language to the extent that they can read and understand printed regulations, detailed written orders, operating procedures, training instructions and materials.
4. Contractor’s personnel shall be fully clothed with trousers, shirts, and shoes, and will bear visible, on their person, identification of both the company name and individual’s name.
a. The Contract Officer Representative or their designee reserves the right to determine the acceptability of any clothing worn.
b. All Contractor personnel working under this contract shall wear company clothing with a distinctive company logo attached in a prominent place on an outer garment.
4. The Contractor shall provide a full-time supervisor who shall be always present on the premises and job site while work is in process for this contract.
5. The supervisor shall be required to speak English for ease in communicating with the EMS Site Manager or his/her designee, the hospital faculty and/or staff.
6. The on-site supervisor shall report to the COR or his/her designee each morning prior to commencement of work to give a progress report of the previous day’s work.
a. Contractor supervisor will report to the COR or their designee, at Building 45 East for daily briefing when performing work at Olin E. Teague Veterans' Medical Center, 1901 Veterans Memorial Drive, Temple, TX 76504 (Bell County). Work shall start no later than 8 a.m. and end for the day no later than 4:30 p.m.
b. Contractor supervisor will report to the COR or their designee, at Building 226 for daily briefing when performing work at Doris Miller Department of Veteran Affairs Medical Center, 4800 Memorial Drive, Waco, TX 76711(McLennan County). Work shall start no later than 8 a.m. and end for the day no later than 4:30 p.m.
c. The contractor supervisor will provide a daily report of that day’s progress and address any issues to the COR.
d. Contractor shall ensure that all Contractor’s Personnel have received proper safety training, appropriate to their respective jobs.
e. The Contractor shall implement an ongoing training program that refreshes the safety skills, qualifications and awareness of the Contractor and Owners workforce safety on a regular, periodic basis.
D. CONTRACTOR PERSONNEL CONDUCT:
1. Contractor personnel shall conduct themselves in a proper, efficient, courteous, and businesslike manner and in accordance with the Central Texas Veterans Health Care System rules and regulations.
2. The use or commutation of alcoholic beverages or controlled substances is strictly prohibited.
3. Contractor shall not permit any person to operate a motor vehicle or heavy equipment while taking prescription or no-prescription medication that may impair their ability to operate said equipment/vehicles in a safe manner.
4. All Central Texas Veterans Health Care System facilities are SMOKE FREE. No smoking or use of tobacco products is allowed on any property.
5. Contractor personnel shall be courteous to all patients, visitors, and employees.
6. Unacceptable behavior on the part of the Contractor’s personnel anywhere may lead to the identifiable Contractor’s personnel being removed from the jobsite.
7. Repeated incidents of unacceptable behaviors on the part of the contractor may be grounds for termination of the contract.
8. Personal grooming, personal hygiene and language of Contractors must be always conducted in a professional manner. Use of foul and off-color language will not be tolerated and can result in Contractor removal from jobsite.
9. Shirts must have sleeves of at least 3 inches, measured from the armpit seam. All buttoned shirts must be buttoned to at least the third button from the top. T-shirt styles may be approved if they are part of a company uniform or contain no political or offensive language or images.
10. Pants that are excessively loose, work excessively low, torn, ragged or with dragging cuffs will not be permitted, Shorts are not permitted.
11. No clothing, accessories, or hardhat stickers that display offensive, derogatory, or inflammatory working or graphics, as determined by the Owner, shall be worn on the jobsite.
12. All clothing must be in good repair and free of any holes or damage. All clothing must be clean and sanitary at the start of each work shift.
E. PERMITS AND LICENSES:
1. The Contractor shall obtain all required permits, licenses, authorizations, and insurance certificates to perform work under this contract and comply with all the applicable federal, state, and local lows and regulations. The contractor shall provide evidence of such permits, certificates, and licenses to the Contracting Officer Representative of their designee before work commences and at other times as requested by the Contracting Officer Representative or their designee.
F. PROTECTION OF PROPERTY:
1. The contractor shall be solely responsible, always, while at the work site for the complete protection of the Temple and Waco medical center locations personnel, property, equipment, and buildings, as well as their own personnel, property, and equipment from injury and/or damage.
2. The contractor shall provide all types of shielding, barricades, warning sign and any other protection necessary to protect the personnel, equipment, and the area in which they are working, from injury, damage, and/or contamination during the full term of the Contract.
3. Protection shall be installed at the beginning of the work period and removed as soon as the work is completed unless permission is given by the Contracting Officer Representative or their designee to leave the protection in place for future work.
G. DAMAGES:
1. Any damages caused by contractor employees to the glass windowpanes; shall be repaired or replaced to the satisfaction of the COR or his/her designee within twenty-four (24) hours from the time of the reported damage.
2. The contractor shall also repair or replace any damage to any Government real property or vegetation to the satisfaction of the COR not later than the close of the next business day.
3. Window screws shall be run up completely to ensure security of the screens. The contractors shall be billed for any screws left off, or not properly installed which will require re-installation by VA Maintenance and Operations employees.
4. Any Government property (glass, lights, screens, drapes, sills, etc.….) damaged by the contractors’ personnel shall be replaced by the contractor or at the contractor’s expense within 24 hours after the damage occurs. The COR or designee shall have final approval of all repairs and/or replacements due to damage. Prior to commencement of work, the contractor shall make a visual inspection of each area to clean, for that day. All existing damages shall be reported to the designated COR or his/her designee.
H. REPAIRS:
1. Visible damage, not caused by the contractor, shall be reported to COR. Failure to report any damage shall result in the contractor being responsible for the damages.
2. Screens and Gratings:
a. Screens shall be placed back in their original positions and secured after cleaning.
b. The contractor shall replace any grating removed from the light wells by the contractor.
I. EQUIPMENT:
All equipment used by the contractor in the performance of the contract, especially the outside work, shall conform to all current Federal, State, and local safety requirements and shall be inspected by VA Safety Officer prior to use on Government Property.
J. ACCESS TO WORK AREAS: The Contractor shall make all arrangements through the Contracting officer Representative or their designee to obtain access to buildings, facilities, and other work areas.
K. WORK CONTROL: The contractor shall implement all necessary work control procedures to ensure timely accomplishment of work requirements, as well as to permit tracking and reporting of work in progress.
1. The contractor shall plan and schedule work to assure material, labor, and equipment are available to complete work requirements within the specified time limits and in conformance with the quality standards established herein.
2. Verbal scheduling and status reports shall be provided when requested by the Contracting Officer Representative or designee.
3. A written status report of any item of work must be provided within 24 hours of the inquiry during regular working hours.
L. SCHEDULE OF CLEANING (TABLE A): Window washing services will be performed as indicated in “Table A” for each CTVHCS location listed in SOW.
M. COMMENCEMENT OF WORK: Contractor shall be required to commence with the work required under this SOW not later than seven (7) days after notification of contract award.
N. SCHEDULE OF WORK: The contractor shall submit a schedule of events /flow chart showing how each phase of the contract will be completed. (e.g.: floor / building, number of employees, type of equipment, cleaning supplies, uniforms, etc.) Work on this contract shall be performed only during administrative hours! These times are Monday through Friday 8:00 a.m. to 4:30 p.m. No work shall be performed on weekends or federal holidays without prior arrangements with the COR. The COR will authorize the exceptions.
O. FEDERAL HOLIDAYS:
1. Also included would be any other day specifically declared by the President of the United States to be a National Holiday.

2. When a holiday falls on a Sunday, the following Monday shall be observed as a legal holiday by U.S. Government agencies. When a holiday falls on a Saturday, the preceding Friday shall be observed as a legal holiday.
P. QUALITY MANAGEMENT SYSTEMS (QMS):
1. The Contractor shall establish and maintain a complete Quality Control Program in accordance with the provisions specified herein. The Contractor’s QC Program shall provide an effective and efficient means of identifying and correcting problems throughout the entire scope of operations. The Contractor’s QC program shall address:
a. Accurate documentation of work processes, procedures, and output measures.
b. A systematic procedure for assessing compliance with performance objectives and standards.
c. Accurate documentation of quality inspections conducted throughout the execution of work.
d. Assessment-driven corrective actions and process adjustments as appropriate in a timely manner.
Q. QUALITY CONTROL INSPECTION: The contractor shall establish and maintain an inspection process to ensure that the work performed conforms to the contract requirements.
1. The Contractor shall maintain a file of all scheduled and performed quality Control Inspections.
2. The file shall be the property of the Owner and make available during the owners regular working hours.
3. The file shall be turned over to the Contracting Officer Representative or their designee within five calendar days of completion or termination of the contract.
R. SITE VISIT: Due to the complexity of scheduling, the two separate locations and the total number of windows involved, a site visit is highly recommended to provide an accurate quote.
S. CHANGES TO STATEMENT OF WORK: Any changes to this SOW shall be authorized and approved only through written correspondence from the Contracting officer (CO). A copy of each change will be kept in a project folder along with all other products of the project. Costs incurred by the contractor through the actions of parties other than the CO shall be borne by the contractor.

(d) Comptroller General Examination of Record. (1) The Comptroller General of the United States, or an authorized representative of the Comptroller General, shall have access to and right to examine any of the Contractor's directly pertinent records involving transactions related to this contract.
(2) The Contractor shall make available at its offices at all reasonable times the records, materials, and other evidence for examination, audit, or reproduction, until 3 years after final payment under this contract or for any shorter period specified in FAR Subpart 4.7, Contractor Records Retention, of the other clauses of this contract. If this contract is completely or partially terminated, the records relating to the work terminated shall be made available for 3 years after any resulting final termination settlement. Records relating to appeals under the disputes clause or to litigation or the settlement of claims arising under or relating to this contract shall be made available until such appeals, litigation, or claims are finally resolved.
(3) As used in this clause, records include books, documents, accounting procedures and practices, and other data, regardless of type and regardless of form. This does not require the Contractor to create or maintain any record that the Contractor does not maintain in the ordinary course of business or pursuant to a provision of law.

C.3 52.217-8 OPTION TO EXTEND SERVICES (NOV 1999)
The Government may require continued performance of any services within the limits and at the rates specified in the contract. These rates may be adjusted only as a result of revisions to prevailing labor rates provided by the Secretary of Labor. The option provision may be exercised more than once, but the total extension of performance hereunder shall not exceed 6 months. The Contracting Officer may exercise the option by written notice to the Contractor within 30 days after contract expiration.

C.4 52.217-9 OPTION TO EXTEND THE TERM OF THE CONTRACT (MAR
2000)
(a) The Government may extend the term of this contract by written notice to the Contractor within 30 days after contract expiration; provided that the Government gives the Contractor a preliminary written notice of its intent to extend at least 60 days before the contract expires. The preliminary notice does not commit the Government to an extension.
(b) If the Government exercises this option, the extended contract shall be considered to include this option clause.
(c) The total duration of this contract, including the exercise of any options under this clause, shall not exceed five (5) years.
C.5 SUPPLEMENTAL INSURANCE REQUIREMENTS
In accordance with FAR 28.307-2 and FAR 52.228-5, the following minimum coverage shall apply to this contract:
(a) Workers' compensation and employers liability: Contractors are required to comply with applicable Federal and State workers' compensation and occupational disease statutes. If occupational diseases are not compensable under those statutes, they shall be covered under the employer's liability section of the insurance policy, except when contract operations are so commingled with a Contractor's commercial operations that it would not be practical to require this coverage.
