Combined Synopsis-Solicitation for Commercial Items Request for Quotation (RFQ) 36C25522Q0099
Description: Services, Non-Personal: On-Site Sleep Study Services
On-Site Sleep Studies – Scheduling, Managing and Monitoring and Scoring, and On-Site Scoring of Home Sleep Apnea Tests (HSAT) for the Robert J. Dole VA Medical Center Medicine Service in Wichita, Kansas.
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested, and a written solicitation document will not be issued. This solicitation is issued as a Request for Quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2022-03.
This procurement is for On-Site Sleep Study Services and is solicited in accordance with FAR Subparts 12.6 and 13.5 for a base 1 year and 4 1-year options contract. The Government anticipates award of a single Fixed Price Indefinite Delivery Indefinite Quantity Contract. This is a Set-aside for Small Business using Tiered, Comparative Evaluations. The NAICS code is 621498, with a small business size standard of $22.0 million, and the PSC is Q403.
Network Contracting Office (NCO) 15, 3450 S. 4th St. Trafficway, Leavenworth, Kansas 66048 seeks to purchase sleep study services for performance 365 days per year, and some daytime services.
Services to be performed include services, but are not limited to: contractor receipt of sleep study needs from clinical personnel, scheduling/re-scheduling with patients and/or their caregivers/family, receipt of patient with instruction/direction, data collection, study-type selection and adjustment/change where appropriate, equipment set-up, computer/network operation, patient and airway monitoring / management, patient-services surveys, records documentation and reporting, discharge/exit, contractor-provided Registered Polysomnographic Technologists (RPSGTs) to perform the studies and scoring. Services include serving up to (regularly per required scheduling) 2 sleep study patients per night. Some daytime studies may be required and total scheduled patients could be 4 in a “day/night” cycle to be served, depending on VA needs and contractor scheduling management.
NOTE: study interpretation and diagnosis does not apply.
The intended contract period is a Base One (1) Year and Four (4) Optional Years that may be exercised.
Frequency of Services: Nightly with Some Daily, 365 Days Per Year, Including All Weekends and Holidays.
Issue Date: 1/14/2022 I interested Small Business firms are solicited to provide quotations for the following Services:

4 1-year option periods may be exercised at the sole discretion of the Government.
Place of Performance: Robert J. Dole VAMC Medicine Service Sleep Center, 5500 E. Kellogg Dr., Wichita, Kansas 67218.
Individual Task Orders will be issued for each annual period of performance, contingent on availability of VA Funding.
SOLICITATION NOTICE OF TIERED EVALUATIONS
This procurement is set-aside based on an order of priority as established in 38 U.S.C. 8127.
TIERED EVALUATIONS EXCLUDING LARGE BUSINESS CONCERNS for NAICS 621498:
This solicitation is being issued as tiered evaluation with the following tiers: (1) SDVOSB concerns, (2) VOSB concerns, (3) Women Owned small business concerns, HUBZone small business concerns and 8(a) participants having priority; and (4) small business concerns not in any other small business category. If award cannot be made, the solicitation will be cancelled, and the requirement resolicited.
(End of Provision) FAR provisions and clauses may be accessed at www.acquisition.gov/comp/far/index.html - The following solicitation provisions apply to this RFQ:
1. FAR 52.212-1 Instructions to Offerors–Commercial Items (Nov 2021) applies, See Attachment 3, Addendum to 52.212-1.
2. FAR 52.212-3, “Offerors Representations and Certifications–Commercial Items” (Nov 2021) applies.
Quoter Registration in www.sam.gov (SAM) is required to be complete at time of quote submission.
Offerors must complete annual representations and certifications electronically via SAM in accordance with FAR 52.212-3. If paragraph (j) of the provision is applicable, a written submission is required.
3. VAAR 852.273-73 “Evaluation – Health-Care Resources (Jan 2003)” (in lieu of FAR 52.212-2) applies, with Addendum to 852.273-72, See Attachment 4.
FAR and VAAR provisions and clauses may be accessed electronically at:www.acquisition.gov/comp/far/index.html and www.acquisition.gov/vaar The following provisions and clauses apply to this RFQ:
• FAR 52.212-4, “Contract Terms and Conditions–Commercial Items (Oct 2018)” applies, with Addendum to 52.212-4, See Attachment 5.
• FAR 52.212-5, “Contract Terms and Conditions Required to Implement Statutes or Executive Orders (Jan
2022” applies, See Attachment 5 and with applicable subparagraphs of 52.212-5.
o Attachment 5 includes additional FAR and VAAR clauses referenced in the addendum.
All quoters shall submit the following: See Instructions to Offerors/Quoters, Attachment 3 All quotes shall be sent to the Contracting Officer listed below and in Attachment 4 Award will be based upon evaluation stated in Attachment 4.
The following are the decision factors: As stated in Attachment 4 The award will be made to the response most advantageous to the Government.
Responses should contain your best terms, conditions.
To facilitate the award process, all quotes must include a statement regarding the terms and conditions herein as follows:
"The terms and conditions in the solicitation are acceptable to be included in the award document without modification, deletion, or addition."
OR
"The terms and conditions in the solicitation are acceptable to be included in the award document with the exception, deletion, or addition of the following:" Quoters shall list exception(s) and rationale for the exception(s), if any.
Submission of your response shall be received not later than January 24, 2022, 10:00 a.m. Central Time by e-mail (below).
Late submissions shall be treated in accordance with the solicitation provision at FAR 52.212-1(f).
