Ultrasound System

Veterans Affairs Health Care System Oklahoma City, Oklahoma

1. General: Currently, the Oklahoma City Veterans Affairs Health Care System (herein after referred to as OKC VAHCS) Surgery Service requires 1 ultrasound system for a new community-based outpatient clinic. To expand orthopedic services, the facility is looking to acquire a GE Venue Go R5 system. This equipment can be used in place of an MRI. It is crucial to get the replacement units delivered to the facility no later than 7 days after issuance of the purchase order.

2. Delivery Location: Oklahoma City Veterans Affairs Health Care System, 50 N.E.

42nd St. Oklahoma City, OK 73105

3. Delivery Times: Delivery times shall be Monday through Friday from 07:00 to 16:00 (7:00AM to 4:00PM). The volume of this order may dictate that hours be extended to 24 hours a day, 7 days per week to meet the needs of the VA. The VA is prepared to support the schedule necessary. Vendor must evaluate labor and volume and provide a schedule to show that they can support a two week, or less installation.

The Government recognized US holidays are: New Year’s Day, Martin Luther King, Jr. Day, Presidents Day, Memorial Day, Juneteenth Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day and Christmas Day. If the holiday falls on Sunday, it is observed on Monday.

4. Definitions/Acronyms: OKC VAHCS: Oklahoma City Veterans Affairs Health Care

System.

5. Scope: The Contractor will provide the necessary equipment and accompanying software to the OKC VAHCS for one (1) ultrasound system. The system must be GE Healthcare Venue Go R5 Ultrasound Imaging System, with transducers as specified in section 8 of this requirement. Additionally, the requirement is for a comprehensive and whole solution to the requirement. The Contractor is required to be an expert in the field of ultrasound imaging systems, specifically within the field of ultrasounds. As such, the Contractor is required to include any items not specifically listed in the pricing schedule but is necessary to enable the equipment or accompanying software to function. Examples include installation expenses, VA staff training, equipment mounts, display charts, cables, connectors, plugs, and special equipment wiring requirements. At minimum, the contractor shall provide a 12 month warranty at no cost to the VA. The company shall coordinate with OKC VAHCS Biomedical Engineering Department to coordinate any preventive maintenance or replacement of any machine.

6. Training for VA Employees: Comprehensive training or VA staff is required. Examples include: Physicians, Nurses, Surgical Technicians, and Biomedical Engineers.

7. Security Requirements: Security clauses are not required due to their being no VA sensitive information is involved.

STATEMENT OF WORK (SOW)

GENERAL PURPOSE / OBJECTIVE:

Eastern Oklahoma VA Health Care System (EOVAHCS) requires a contractor to provide the central venous access ultrasound systems to guide the insertion of a central venous catheter (CVC), a tube inserted into a large vein near the heart. This real-time guidance helps improve the success rate of placement, reduces the number of attempts needed, and minimizes complications like arterial puncture or pneumothorax. The contractor will be required to supply these items in accordance with this statement of work and in a cost effective, efficient, and safe manner, while in accordance with the VA Mission.

BACKGROUND: The Eastern Oklahoma VA Health Care System (EOVAHCS) is renovating a non-medical existing facility to be the new Tulsa VA Medical Center (VAMC). This Activation project requires new central venous access ultrasound for use in Intensive Care Unit. Currently none of this equipment is in this facility. Delivery is to occur to an offsite warehouse so that it can be installed once the facility is under VA control. Installation is NOT included in this SOW.

LOCATION: Product Delivery: Jack C. Montgomery VA Medical Center, 1011 Honor Heights Dr, Muskogee, OK 74401

Each delivery package must be MARKED, “FOR: PO#” (insert PO# from Award) and

“PKG258”

DELIVERY TIMES (PERFORMANCE PERIOD): The VA requires delivery 30 days ARO, no later than September 4th 2025. Will be drop shipped and no installation required.

SCOPE: EOVAHCS requires a contractor to provide products and assembly that meet or exceeds the physical, functional, and performance of the listed salient characteristics in the following sections. The equipment features are essential to the technical requirements to meet the VA’s needs. The VA’s needs assessment identified the type of products and components listed below as a suitable solution to fill the gap. However, these are only used as a guide and the VA’s basic, salient characteristics and critical minimum requirements define the type of product that is needed to ensure the Purpose of this SOW is satisfied. OEM part numbers/references, in the salient characteristics sections are used as a basis of design and are provided as a means to convey the minimum standard characteristics.

The needle tracking system holder contains an RFID reader. Irrigation system must be easy to set up and use, have an ergonomic grip, and promote efficient turnover of the operating room.

WARRANTY OF EQUIPMENT: Contractor must provide a warranty per the Specifications in this SOW. The warranty must meet or exceed standard manufacturer industry warranty terms. The government does not accept an extended warranty, which requires advance payment. Manufacturer’s warranty must begin on 1st Patient Day, which is currently scheduled for January 20th, 2026.

SECURITY REQUIREMENTS: The C&A requirements do not apply, and that a Security Accreditation Package is not required. Contractor must specify the mechanism/documentation used to ensure the VA sensitive information is protected.

(d) Comptroller General Examination of Record. The Contractor shall comply with the provisions of this paragraph (d) if this contract was awarded using other than sealed bid, is in excess of the simplified acquisition threshold, as defined in FAR 2.101, on the date of award of this contract, and does not contain the clause at 52.215-2, Audit and Records—Negotiation.

(1) The Comptroller General of the United States, or an authorized representative of the Comptroller General, shall have access to and right to examine any of the Contractor's directly pertinent records involving transactions related to this contract.

(2) The Contractor shall make available at its offices at all reasonable times the records, materials, and other evidence for examination, audit, or reproduction, until 3 years after final payment under this contract or for any shorter period specified in FAR Subpart 4.7, Contractor Records Retention, of the other clauses of this contract. If this contract is completely or partially terminated, the records relating to the work terminated shall be made available for 3 years after any resulting final termination settlement. Records relating to appeals under the disputes clause or to litigation or the settlement of claims arising under or relating to this contract shall be made available until such appeals, litigation, or claims are finally resolved.

(3) As used in this clause, records include books, documents, accounting procedures and practices, and other data, regardless of type and regardless of form. This does not require the Contractor to create or maintain any record that the Contractor does not maintain in the ordinary course of business or pursuant to a provision of law.

C.3 VAAR 852.232-72 ELECTRONIC SUBMISSION OF PAYMENT

REQUESTS (NOV 2018)

(a) Definitions. As used in this clause—

(1) Contract financing payment has the meaning given in FAR 32.001;

(2) Designated agency office means the office designated by the purchase order, agreement, or contract to first receive and review invoices. This office can be contractually designated as the receiving entity. This office may be different from the office issuing the payment;

(3) Electronic form means an automated system transmitting information electronically according to the accepted electronic data transmission methods and formats identified in paragraph (c) of this clause. Facsimile, email, and scanned documents are not acceptable electronic forms for submission of payment requests;

(4) Invoice payment has the meaning given in FAR 32.001; and

(5) Payment request means any request for contract financing payment or invoice payment submitted by the contractor under this contract.

(b) Electronic payment requests. Except as provided in paragraph (e) of this clause, the contractor shall submit payment requests in electronic form. Purchases paid with a Government-wide commercial purchase card are considered to be an electronic transaction for purposes of this rule, and therefore no additional electronic invoice submission is required.

(c) Data transmission. A contractor must ensure that the data transmission method and format are through one of the following:

(1) VA’s Electronic Invoice Presentment and Payment System at the current website address provided in the contract.

(2) Any system that conforms to the X12 electronic data interchange (EDI) formats established by the Accredited Standards Center (ASC) and chartered by the American National Standards Institute (ANSI).

(d) Invoice requirements. Invoices shall comply with FAR 32.905.

(e) Exceptions. If, based on one of the circumstances in this paragraph (e), the Contracting Officer directs that payment requests be made by mail, the Contractor shall submit payment requests by mail through the United States Postal Service to the designated agency office.

Submission of payment requests by mail may be required for—

(1) Awards made to foreign vendors for work performed outside the United States;

(2) Classified contracts or purchases when electronic submission and processing of payment requests could compromise the safeguarding of classified or privacy information;

(3) Contracts awarded by contracting officers in the conduct of emergency operations, such as responses to national emergencies;

(4) Solicitations or contracts in which the designated agency office is a VA entity other than the VA Financial Services Center in Austin, Texas; or

(5) Solicitations or contracts in which the VA designated agency office does not have electronic invoicing capability as described above.

I. OFFER SUBMITTAL INSTRUCTIONS:

1) Offeror must complete and return the Price Schedule along with the highlighted portions of Provisions 52.212-3 and 52.209-7.

2) Contract pricing and Delivery period: All pricing shall be invoiced based on Price Schedule and the Salient Characteristics.

3) Offeror shall submit information supporting past performance in providing , accessories as listed in the Statement of Work.

4) Subject line of e-mail shall be “Quote – 36C24W25Q0384- Ultrasound Systems”

5) Offeror must fill out FAR 52.225-2 Buy American Certificate (attached) clearly and correctly for quote to be considered.

II. Pricing: Contractor shall submit pricing, which incorporates all the requirements within the Price Schedule and Statement of Work.

III. Period for acceptance of offers: The offeror agrees to hold the prices in its offer firm for 30 calendar days from the date specified for receipt of offers.

Failure to submit a complete offer may result in an unacceptable determination. Late submissions will be set-aside as unacceptable and will be evaluated only if it benefits the Government.

IV. Basis of Award

The Government intends to award a purchase order resulting from this solicitation to the vendor, who can provide the supplies to meet this requirement. This is a competitive comparative analysis selection in which competing offeror’s past performance, technical and price will be evaluated to determine who represents the greatest benefit to the Government at a price that can be determined fair and reasonable.

By submission of its offer, the offeror accedes to all solicitation requirements, including terms and conditions, representations and certifications, and technical requirements. Failure to meet a requirement or inability to perform or provide all goods and services requested shall result in an offer being determined technically incapable. Offerors must clearly identify any exception to the solicitation terms and conditions and provide complete accompanying rationale. The evaluation process shall proceed as follows:

A. Following receipt of quotes, the Government will perform a comparative analysis of the goods and services quoted. The Government will compare quotes to one another to select the vendor that best benefits the Government in fulfilling the requirement. The evaluation of Government requirements outlined in the Request for Quote will determine suitability.

B. The Government will evaluate quotes for award purposes using the following factors:

1) Past Performance

2) Technical Capability

3) Price

Factor 1: Past Performance – The contractor shall provide the following documentation with their offer:

Two (2) references from comparable Government contracts or successful commercial contracts for Ultrasound Systems.

Factor 2: Technical Capability – The Contractor shall provide the following documentation with their offer:

Capability Statement, maximum 2 pages which can validate Technical Capability: Capability statement should include a description of technical approach, dimensions to ensure the proper fit, and techniques to provide the salient characteristics promptly and expertly.

Factor 3: Price – The Contractor shall provide the following documentation with their offer:

Pricing: Contractor shall submit unit prices rounded to the nearest hundredth. Prices should be documented on the Price/Cost Schedule. Please ensure unit price and total amounts are correct.

Period for acceptance of offers: The offeror agrees to hold the prices in its offer firm for 30 calendar days from the date specified for receipt of offers.

Prices will be evaluated for completeness and reasonableness. Unrealistically high or low proposed costs or prices, initially or subsequently, may be grounds for eliminating an offeror from competition.

Additionally, unbalanced pricing poses an unacceptable risk to the Government and may be reason to reject an offer. The burden of proof for credibility of proposed costs/prices rests with the offeror. The Government reserves the right to amend the solicitation and remove any set-aside should pricing of all quotes be found unreasonable.
