STATEMENT OF WORK (SOW)
1. Contract Title.
Sound suite/Audio booth-new Mt Vernon CBOC ACTIVATION
2. Background.
The VA Puget Sound Health Care System is seeking to procure and install an audiology booth at the new Mt Vernon VA Clinic. The new clinic is being built and will require a booth for Audiology services to continue. The audiology booth will provide a double soundproof suite for patients and employees to conduct hearing exams in an isolated area for precise results. The primary function of the audiology booth is to provide an isolated acoustical room to measure hearing acuity in patients. Isolated acoustic rooms are crucial to eliminating background noise to acquire a reliable diagnosis in patients. Audiology booth will be sunk into a pit so it is flush with surrounding floor. Key criteria include system size, weight, and acoustic install.
3. Scope.
Purchase and install an audiology booth in New Mt Vernon CBOC. See specs attached as it must meet these requirements.
一 The contractor shall deliver and install the system with all specified salient characteristics. Only new equipment will be accepted.
一 The contractor shall provide shipping, transport, installation, and all equipment necessary to meet the requirements and capabilities listed within the scope.
一 The contractor shall deliver the audiology booth to the New Mt Vernon facility as specified below.
一 The assembly and validation of the system must be performed by a vendor service trained representative.
一 The contractor is responsible for identifying any missing parts or components not included in the order for the audiology booth to be successfully installed and validated to their full potential.
3.1. SALIENT CHARACTERISTICS: Please note, all items listed below must be technically equivalent to ETS Lindgren Audiology Booth.

一 Unit provides recessed incandescent lighting.
一 Unit must have an acoustic door with cam -lift hinges, providing at least 36” clear width opening.
一 Must include a double glazed, safety glass window that is at least 6 mm (1/4”) thick and at least 30” x 24”.
一 Unit must be electrically serviced via a grounded cable, having a length of at least 20 feet.
一 Unit floor must be acoustically isolated in the exam room.
一 Unit must have a noise reduction coefficient of 1.05, while having an absorption coefficient range of 0.79-1.10 when measuring the center frequency (Hz) between a range of 125-400 Hz.
一 Door size must be at least 36” W with clear opening.
一 Door handles must be a C-shape while providing a minimum clearance of 1 ½” behind it.
一 Doors cannot require more than 5psi constant pressure to open, once the magnetic seal is broken.
一 Double glazes acoustic windows for the door on the booth, size options must include:
20” W by 60” H.
一 Unit includes a standard fan exhaust and fan inlet silencers suspended to the ceiling.
Silencers mist be capable of 10 to 12 full air exchanges per hour. Fans include a 6” or 8” diameter HVAC collar option.
一 Unit must include two ventilation silencers built into the booth’s ceilings, one supply and one exhaust provided.
一 Unit must include an accessible jack panel that provides at least 12 (TRS 3-wire), ¼” phone jacks, two USB ports, and 1 ¾” pass through. Panel face must be removable and does not impact the rooms acoustical performance.
一 Sprinkler holes must be at least 3” in diameter.
一 Unit must by supplied with standard carpet or carpet tile.
一 Must be able to provide empty utility boxes with pull wires for data, thermostats, and alarms.
一 Unit must provide at least 2 duplex outlets and light/fan switches.
一 Unit wall panels must be at least 8” thick.
一 Unit ceiling panels must be at least 8” thick.
一 Light switches must be at least 44” from the floor inside of the unit, in both rooms.
一 Outlets must be at least 15” from the floor inside of the unit, in both rooms.
一 Must include rubber mat as floors inside the booth at a height no greater than 10mm (3/8”) height for wheelchair access.
一 Booth doors must be at least 75 mm (2 7/8”) thick.
一 Booth doors mounted with heavy duty cam lift hinges.
一 Door sealing system must have at least a double magnetic gasket sealing system and flush sill seal design for an accurate acoustic seal.
一 Panels in the unit must have a Flame Spread rating of 3.
一 Panels in the unit must have a Smoke Density Rating of 2.
一 must come with standard sprinkler penetrations for onsite installation.
一 All electrical in the unit must be Hospital Grade.
一 Panels must interlock and provide an acoustic seal.
3.2. IMPLEMENTATION AND INSTALLATION Installation and validation shall be included in the procurement cost and shall include but not limited to: Assembly of devices and validation.
一 All materials required to assemble the audiology booth shall be provided by the contractor.
一 The contractor will deliver the audiology booth and accessories to the addresses below:
New Mt Vernon CBOC Audiology Booth and Accessories.
3.3 TRAINING The contractor shall provide onsite training for all VAMHCS staff that requires training.
3.4 DELIVERY The contractor shall deliver necessary equipment to the New Mt Vernon VA facility as required in the quantity specified above.
1 The contractor must communicate with the contractor to install booth.
2 The contractor must give VAPSHCS staff 3 weeks’ notice of shipment and delivery of the audiology booth and accessories.
3 The contractor shall deliver equipment and all items required for installation only when the site is ready for installation to proceed. Installation can proceed 90 days after award of contract.
4 The contractor shall coordinate all deliveries, installation, and parking arrangements with the local VA POC.
5 Delivered materials will arrive in manufacturer’s original sealed containers with the associated purchase order number listed on the outside of the box.
6 The contractor shall conduct a joint inspection with the local VA POC upon delivery of the equipment and shall provide the local VA POC a list of missing or damaged items found upon inspection.
7 The contractor shall remove all related shipping debris and packing from the VAPSHCS facility.
4. Performance Monitoring (if applicable).
Local VA POC - Emily Hanson (or Audiology representative) will be present at initial, during, and end of install.
5. Security Requirements.
The contractor employees shall not have access to VA sensitive or computer information and will not require routine access to VA Facilities. The contractor employees shall require intermittent access only and will be escorted by VA employees while at VA Facilities. No background investigation is required
6. Government-Furnished Equipment (GFE)/Government-Furnished Information (GFI).

(d) Comptroller General Examination of Record. (1) The Comptroller General of the United States, or an authorized representative of the Comptroller General, shall have access to and right to examine any of the Contractor's directly pertinent records involving transactions related to this contract.
(2) The Contractor shall make available at its offices at all reasonable times the records, materials, and other evidence for examination, audit, or reproduction, until 3 years after final payment under this contract or for any shorter period specified in FAR Subpart 4.7, Contractor Records Retention, of the other clauses of this contract. If this contract is completely or partially terminated, the records relating to the work terminated shall be made available for 3 years after any resulting final termination settlement. Records relating to appeals under the disputes clause or to litigation or the settlement of claims arising under or relating to this contract shall be made available until such appeals, litigation, or claims are finally resolved.

C.3 VAAR 852.203-70 COMMERCIAL ADVERTISING (MAY 2018)
The Contractor shall not make reference in its commercial advertising to Department of Veterans Affairs contracts in a manner that states or implies the Department of Veterans Affairs approves or endorses the Contractor’s products or services or considers the Contractor’s products or services superior to other products or services.
C.4 VAAR 852.215-70 SERVICE-DISABLED VETERAN-OWNED AND
VETERAN-OWNED SMALL BUSINESS EVALUATION FACTORS (JAN 2023)
(DEVIATION)
(a) In an effort to achieve socioeconomic small business goals, VA shall evaluate offerors based on their service-disabled veteran-owned or veteran-owned small business certification status and their proposed use of eligible certified service-disabled veteran-owned small businesses (SDVOSBs) and certified veteran-owned small businesses (VOSBs) as subcontractors.
(b) Eligible and certified service-disabled veteran-owned small businesses offerors will receive full credit, and offerors qualifying as veteran-owned small businesses will receive partial credit for the certified Service-Disabled Veteran-Owned and Veteran-Owned Small Business Status evaluation factor. To receive credit, an offeror must be listed at the time of submission of offers and at time of award, as a certified SDVOSB/VOSB in the SBA certification database on the SBA Veteran Small Business Certification Program portal at https://veterans.certify.sba.gov/ (see 13 CFR 128) and be otherwise eligible in accordance with SBA size standards for the acquisition and limitations on subcontracting requirements set forth in SBA regulations (see 13 CFR 121, 125 and 128.).
(c) Non-Veteran offerors proposing to use certified SDVOSBs or VOSBs as subcontractors will receive some consideration under this evaluation factor. Offerors must state in their proposals the names of the certified SDVOSBs and VOSBs with whom they intend to subcontract and provide a brief description of the proposed subcontracts and the approximate dollar values of the proposed subcontracts. In addition, the proposed subcontractors must be listed at the time of submission of offers and at time of award, as a certified SDVOSB/VOSB in the SBA certification database.

C.6 VAAR 852.232-72 ELECTRONIC SUBMISSION OF PAYMENT
REQUESTS (NOV 2018)
(a) Definitions. As used in this clause—
(1) Contract financing payment has the meaning given in FAR 32.001;
(2) Designated agency office means the office designated by the purchase order, agreement, or contract to first receive and review invoices. This office can be contractually designated as the receiving entity. This office may be different from the office issuing the payment;
(3) Electronic form means an automated system transmitting information electronically according to the accepted electronic data transmission methods and formats identified in paragraph (c) of this clause. Facsimile, email, and scanned documents are not acceptable electronic forms for submission of payment requests;
(4) Invoice payment has the meaning given in FAR 32.001; and
(5) Payment request means any request for contract financing payment or invoice payment submitted by the contractor under this contract.
(b) Electronic payment requests. Except as provided in paragraph (e) of this clause, the contractor shall submit payment requests in electronic form. Purchases paid with a Government-wide commercial purchase card are considered to be an electronic transaction for purposes of this rule, and therefore no additional electronic invoice submission is required.
(c) Data transmission. A contractor must ensure that the data transmission method and format are through one of the following:
(1) VA’s Electronic Invoice Presentment and Payment System at the current website address provided in the contract.
(2) Any system that conforms to the X12 electronic data interchange (EDI) formats established by the Accredited Standards Center (ASC) and chartered by the American National Standards Institute (ANSI).
(d) Invoice requirements. Invoices shall comply with FAR 32.905.
(e) Exceptions. If, based on one of the circumstances in this paragraph (e), the Contracting Officer directs that payment requests be made by mail, the Contractor shall submit payment requests by mail through the United States Postal Service to the designated agency office.
Submission of payment requests by mail may be required for—
(1) Awards made to foreign vendors for work performed outside the United States;
(2) Classified contracts or purchases when electronic submission and processing of payment requests could compromise the safeguarding of classified or privacy information;
(3) Contracts awarded by contracting officers in the conduct of emergency operations, such as responses to national emergencies;
(4) Solicitations or contracts in which the designated agency office is a VA entity other than the VA Financial Services Center in Austin, Texas; or
(5) Solicitations or contracts in which the VA designated agency office does not have electronic invoicing capability as described above.

C.8 VAAR 852.246-71 REJECTED GOODS (OCT 2018)
(a) Supplies and equipment. Rejected goods will be held subject to Contractor’s order for not more than 15 days, after which the rejected merchandise will be returned to the Contractor’s address at the Contractor’s risk and expense. Expenses incident to the examination and testing of materials or supplies that have been rejected will be charged to the Contractor.
(b) Perishable supplies. The Contractor shall remove rejected perishable supplies within 48 hours after notice of rejection. Supplies determined to be unfit for human consumption will not be removed without permission of the local health authorities. Supplies not removed within the allowed time may be destroyed. The Department of Veterans Affairs will not be responsible for, nor pay for, products rejected. The Contractor will be liable for costs incident to examination of rejected products.

Material shall be packed for shipment in such a manner that will insure acceptance by common carriers and safe delivery at destination. Containers and closures shall comply with regulations of carriers as applicable to the mode of transportation.

I. OFFER SUBMITTAL INSTRUCTIONS:
1) Offeror shall submit offers by Due Date/Time specified in Block 8 of the Standard Form (SF) 1449 of the solicitation directly to Ian Putnam, Contract Specialist, at ian.putnam@va.gov.
2) By signing the Standard Form (SF) 1449/Solicitation, Contractor attests acknowledgement of receipt of all solicitation amendments if applicable.
3) Offeror must complete and return the Price Schedule along with the highlighted portions of Provisions 52.212-3.
4) Contract pricing and Performance periods: All pricing shall be invoiced based on Price Schedule and the Statement of Work.
5) Offeror shall submit information supporting past performance in providing Audiology Booths, accessories as listed in the Statement of Work.

II. Pricing: Contractor shall submit pricing, which incorporates all the requirements within the Price Schedule and Statement of Work.
III. Period for acceptance of offers: The offeror agrees to hold the prices in its offer firm for 30 calendar days from the date specified for receipt of offers.
Failure to submit a complete offer may result in an unacceptable determination. Late submissions will be set-aside as unacceptable and will be evaluated only if it benefits the Government.
IV. Basis of Award
The Government intends to award a purchase order resulting from this solicitation to the vendor, who can provide the supplies to meet this requirement. This is a competitive comparative analysis selection in which competing offeror’s past performance, technical and price will be evaluated to determine who represents the greatest benefit to the Government at a price that can be determined fair and reasonable.
By submission of its offer, the offeror accedes to all solicitation requirements, including terms and conditions, representations and certifications, and technical requirements. Failure to meet a requirement or inability to perform or provide all goods and services requested shall result in an offer being determined technically incapable. Offerors must clearly identify any exception to the solicitation terms and conditions and provide complete accompanying rationale. The evaluation process shall proceed as follows:
A. Following receipt of quotes, the Government will perform a comparative analysis of the goods and services quoted. The Government will compare quotes to one another to select the vendor that best benefits the Government in fulfilling the requirement. The evaluation of Government requirements outlined in the Request for Quote will determine suitability.
B. The Government will evaluate quotes for award purposes using the following factors:
1) Past Performance
2) Technical Capabilities
3) Price
Factor 1: Past Performance – The contractor shall provide the following documentation with their offer:
Two (2) references from comparable Audiology Booths, or with requested equipment or similar equipment, which can validate experience within the last 2 years.
Factor 2: Technical Capability – The Contractor shall provide the following documentation with their offer:
Capability Statement, maximum 2 pages which can validate Technical Capability: Capability statement should include a description of technical approach, methodology and technique to provide the Statement of Work promptly, skillfully, and expertly.
Factor 3: Price – The Contractor shall provide the following documentation with their offer:
Pricing: Contractor shall submit unit prices rounded to the nearest hundredth. Prices should be documented on the Price/Cost Schedule. Please ensure unit price and total amounts are correct.
Period for acceptance of offers: The offeror agrees to hold the prices in its offer firm for 30 calendar days from the date specified for receipt of offers.
Prices will be evaluated for completeness and reasonableness. Unrealistically high or low proposed costs or prices, initially or subsequently, may be grounds for eliminating an offeror from competition either on the basis that the offeror does not understand the requirement or has submitted an unrealistic offer. Additionally, unbalanced pricing poses an unacceptable risk to the Government and may be reason to reject an offer. The burden of proof for credibility of proposed costs/prices rests with the offeror. The Government reserves the right to amend the solicitation and remove any set-aside should pricing of all quotes be found unreasonable.
