STATEMENT OF WORK
Haskell Emergency Generator

1.0 INTRODUCTION / BACKGROUND
The Haskell Indian Health Center was originally constructed in 1978. The Haskell Indian Health Clinic has never installed an emergency generator due to the Health Clinic being classified as business occupancy. However, with the installation of an Emergency Generator, the Haskell Indian Health Clinic will be able to provide continued care and keep vaccines stabilized in house during a power outage. This will provide continuous patient care and keep critical medications and vaccines stabilized.
2.0 OBJECTIVE
The scope of this project is to install a 150KW Emergency Generator and Automatic Transfer Switch (ATS). The Haskell Health Center will be adding electrical circuits for patient care in addition to emergency or critical circuits. The health center will be operational during the installation of the Emergency Generator and ATS.
3.0 SCOPE OF WORK
The scope of this project is to provide a new Emergency Generator and Automatic Transfer Switch (ATS) at the existing Healthcare Center.
1. Install 150KW Emergency Generator
a. Rated watts kW150
b. Rated amps 600A
c. Voltage 120/240 Three Phase
d. Fuel Type Natural Gas
e. UL Listed
f. Liquid Cooling System
g. Low Oil Alert
h. 75 dbA or lower at 25ft.
i. Auto - Start
2. Install Automatic Transfer Switch
a. Power and control wiring, electrical piping and accessories shall be EMT for surface mount areas and PVC to be used underground. ATS shall be properly grounded as per described in the National Electric code.
3. Concrete Pad for Generator
a. Constructed L 15ft x W 8ft Depth 8” thick and minimum 3,500 PSI.
Reinforced with #3 deformed rebar placed at 12” spaces each way and 3” from all edges.
b. 3 Pour in place yellow Safety Bollards - 5 ½” x 36” with anchor tabs, on Northside of concrete pad.
c. Black chain link fencing with entrance gate around concrete pad.
4. Electrical installation and labor
4.0 TASKS / REQUIREMENTS
Contractor shall be responsible for all aspects of preparing area of work for renovation, including to demolition, removal of existing construction materials, and furnishing all labor and materials to perform new work as required.
4.1 General and Administrative
A. Contractor shall provide a full-time superintendent to be on site while supervising, scheduling work and while work is being performed at the health center by the general contractor or their subcontractors.
B. Superintendent shall perform other administrative and supervisory duties as required for proper performance of the Work. Other duties shall include those of Quality Control and Safety Officer Measures during all activities of Work.
C. Contractor shall provide their own Personal Protective Equipment (PPE), Lockout
/ Tag-out equipment, infection control barriers, vehicles barriers, and any other safety related supplies, materials, and equipment to perform this job.
D. Contractor shall provide a dumpster for the removal of debris, material, and waste for the duration of the project and dispose of according to State, Federal, and EPA regulations. At least 50% of recyclable materials shall be recycled.
Location of the dumpster shall be coordinated with the hospital Facility Manager.
E. Contractor shall provide portable chemical toilets as needed for this project.
F. Contractor shall clean work areas daily. Keep work area, and public access areas free from accumulations of waste materials. Contractor shall remove trash daily. Construction waste shall be transported in tightly covered containers to minimize spreading of dirt and debris associated with construction.
G. Upon completion of the work, Contractor shall remove barrier material carefully to minimize the spreading of dirt and debris associated with construction.
5.0 DELIVERABLES
5.1 List of Deliverables

Submitted first draft before Notice to Proceed (NTP), and update within 7-days from NTP. Update as required before each progress meeting.

Contractor shall update list when changes occur.
3 Project Specific Safety Plan Prior to mobilization.
4 Infection Control/Risk Assessment
Coordinate with the Facility Manager. Complete within 1-week after pre-construction meeting.
5 Submittal Register Deliver at pre-construction meeting.
6 Schedule of Values Deliver at pre-construction meeting.
Progress Meetings and Correspondence 7 Progress Reports Daily to COR and CO.
8 Weekly Meeting
Superintendent shall meet with Facility Manager to let him know what work is being planned for the upcoming week so he/she can coordinate with health center staff.
Construction / Installation
9 Update Progress Schedule
Monthly. Provide with progress payments at the monthly progress meetings.
10 Submittals Required.
11 Shop Drawings As required.
12 Request For Information (RFI) As required.
13 Safety Data Sheets (SDS) Required.

5.2 Submittals
Submittals may be shop drawings, product data, cut-sheets of equipment, material samples, sketches, manufacturer’s literature, catalog descriptions, or other description sufficient in detail for review and action to allow a decision by COR. COR shall respond within five (5) business days.
Contractor shall maintain a submittal log with the date submitted for review, the date received back, and action taken.
Substitutions or deviations not approved by the COR risk rejection. Submittals and Safety Data Sheets (SDS) for chemicals and materials shall not be used until approved.
5.3 RFI
If the Contractor desires an explanation or interpretation of the contract or drawings, etc., the request shall be submitted in writing as a Request for Information (RFI) to the COR and CO via electronic format.
Contractor shall maintain a log, numbered sequentially, of all RFIs submitted, the date submitted, and the date an answer was provided. A copy of the RFI log will be provided at the progress meeting for review.
5.4 Warranty
Warranty shall continue for a period of 1-year from the date of final acceptance of the work.
5.5 Transmittals
The contractor shall provide 2 hard copies of each deliverable, and one electronic version.
6 PROGRESS / COMPLIANCE / INSPECTION
The Government requires the following meetings from contractors in order to monitor progress and ensure compliance:
6.1 Progress Meetings
COR will coordinate with Contractor in scheduling and administer the construction progress meetings throughout the Construction.
Weekly meetings shall be held each week with the Facility Manager on site to discuss weekly status, accomplishments, and planned activities for the week, and issues that need coordination.
6.2 Inspections
Government shall inspect regarding quality assurance and remedies regarding non-conforming work.
6.3 Contractor – Signing In/Out
Contractor shall coordinate with the Facility Manager on the signing in/out and scheduling of Contractor’s staff and sub-contractors in order to maintain efficient use of traffic-flow, parking, and access to building and locked areas.
6.4 Life Safety and Infection Control
Before any construction or services are to be performed within the health center, the Contractor is 100% responsible for implementing and maintaining all Safety and Infection Control Measures during performance of their contract.
Initial and periodic inspection will be conducted by the COR, Safety Officer, and/or Infection Control Officer. Any life safety or infection control deficiencies will have to be immediately corrected by Contractor.
Contractor is responsible for implementing all safety and infection control measures to ensure these measures meet OSHA, NFPA Codes, ASHRE, and Joint Commission standards during the course of this project.
6.5 Risk Assessment – Health Center Policy
Contractor shall read and agree to all of the health safety regulations needed for this job.
A. The Health Center Safety Officer will perform the Infection Control Risk
Assessment for Construction and Renovation prior to start of project. This will be reviewed with Contractor prior to start of project.
7 REFERENCES
7.1 Existing As-built drawings are available for review. Coordinate with health center
Facility Manager.
7.2 Quality of Workmanship
Contractor shall perform high quality trade and craft workmanship. All work shall conform to, and comply with, the latest edition and version of ALL applicable codes including, OSHA, NFPA, and IBC, FGI Guidelines for Design and Construction of Health Care Facilities 2014 edition, and Joint Commission Standards.
8 SECURITY REQUIREMENTS
Contractor shall abide by all Safety, Security and Infection Control Policies and Procedures of the Haskell Indian Health Center.
Contractor shall comply with security requirements including personal identification.
Contractor shall be responsible for providing each of their employee with an ID badge.
ID badge shall be worn at all times when performing work during the course of this project.
9 LOCATION
9.1 The project is located at the Lawrence, Kansas, 2415 Massachusetts Street, Lawrence, Kansas 66046.
9.2 Contractor Work Hours
Normal business hours are Monday through Friday from 7:00 a.m. to 6:00 p.m.
Work identified in this scope of work shall be accomplished while the health center is occupied and operational.
Coordination and scheduling of work shall be coordinated with the COR.
Work performed shall be planned to minimize the impact to services to the health center. Working after-hours and weekends shall be coordinated with the local Facility Manager and COR.
9.3 Tobacco / Vape Free Campus
The Haskell Indian Health Center is a tobacco and vape free campus. No smoking or vaping is permitted on the campus grounds.
11 PERIOD OF PERFORMANCE
The period of performance of this scope of work is 60-calendar days after notice to proceed (NTP).
This 60-calendar day includes performance period for mobilization, submittals, material acquisition, installation, inspection, closeout items, and demobilization.
