There are several places to get logs to put in streams. Some of the areas listed are close to the creek. In some cases the logs may not fit that creek or may no longer be viable as a stream log -so the contractor needs flexibility to pick the right logs.

The creeks are listed with number of sites per creek above. Work for the first two creeks shall be performed in the base year or option periods one or two. The last creek listed shall be performed in option period 3.

SECTION C – STATEMENT OF WORK

LM Medford District Office Aquatic Habitat Restoration Statement of Work

GENERAL

Description of Work - The purpose of this contract is to improve riparian habitat for salmonid fish species and other aquatic biota by creating large wood instream structures, removing or modifying nonfunctional instream structures (legacy or irrigation structures), and obliterating or modifying road and other features to reduce sources of sediment.

The following are examples of types of work for which this contract will be used:

Cutting, moving, and placing large woody debris (LWD), and rootwads into stream channels to create aquatic habitat. Transportation of LWD may involve cutting and decking trees in one location and transporting logs up to 70 miles. In some cases, cables may be necessary to yard LWD between locations. LWD will be keyed into structures and standing trees adjacent to stream to reduce the chance they will migrate downstream.

Decommissioning roads with stream crossings, in many cases, recontouring the road surface. Decommissioning will almost always involve removal of all culverts, including crossdrains, and blocking the road by ditching and berming, or blocking with boulders.

Dewatering streams while working on stream channels or removing culverts at stream crossings. Implementing turbidity controls and monitoring release of water to comply with State of Oregon water quality requirements.

Seeding and mulching disturbed ground after projects are completed.

Location of Project Areas - Operations will be confined to lands within the general area of the Medford District BLM.

Project Work Period – Work tasks will occur during instream work timelines (Oregon Guidelines for Timing of In-Water Work to Protect Fish and Wildlife Resources, 2022) between June 15 and September 15 (Rogue). Work outside of the stream and riparian area may occur at any time of the year based on project location, timing, and proximity to Threatened and Endangered species. See Period of Performance and Table l below for Threatened and Endangered species list and applicable work period.

Boundaries of Project Areas -All restoration sites/trees locations will be identified with flagging or paint. Flagging will be hung adjacent to each respective structure site along with corresponding flagging indicating structure's staging areas along road and/or identifying access corridors. Road decommissioning will be identified with a map showing locations of the roads and stream crossings. Flagging maybe used to indicate different objectives along the road segment. Flagging will identify project boundary limits of the disturbance area, within which all activity must occur.

Unit Priority - The sequence of work will be determined by the Government at the pre-work conference and may be subject to change. Sub-items may run concurrently on a project.

Project/Site Conditions/Access - BLM roads will be used for access. Road surface types include black top, gravel roads (improved), dirt roads (unimproved), and access routes.

Some roads will require four-wheel drive vehicles. Seasonal roads may be impassable for several days after heavy rains or snow-blocked during the winter months. In some instances, work may occur on roads owned or maintained by other entities. Contractor will be responsible for ensuring they are complying with any requirements, including bridge weight limitations, and providing necessary traffic control.

Acceptable equipment for aquatic and terrestrial restoration will depend on slope, access, and the presence of threatened or endangered species. In some scenarios stream restoration activities shall require machinery capable of placing wood in streams up to several hundred feet away and without entering the stream channel. In other areas, it will be necessary for tracked equipment to enter the stream channel to complete the tasks. Work shall be completed with the intention of minimizing all ground disturbance and damage to trees by only traveling designated routes, and maneuvering equipment carefully around trees. Any

BLM Medford District Office Page 15 of 79 removal of vegetation to access sites will need to be approved by the Contracting Officer's Representative (COR).

Access to individual project areas may be through locked gates. Access behind locked gates shall be for the purpose of conducting work under this contract only. Locked gates shall remain locked at all times other than when the contractor is passing through.

A key for access to locked gates may be supplied to the contractor at issuance of a task order. The key shall be returned to the COR in a usable condition before final payment is made. The contractor shall pay actual charges for lost, damaged or misplaced keys. The contractor will not make copies of issued keys.

Camping - Work camps may be established in locations with written authorization by the appropriate managing unit. Authorized camps and work sites shall be maintained in a sanitary and orderly condition by clearing the site(s) of all debris, refuse, and garbage before leaving the area. Upon completing the project, the contractor will leave the site in the same condition prior to arrival. Equipment and vehicles should be parked to minimize ground disturbance.

Soil disturbance on roads and project areas shall be minimized. All roadways shall be cleared of woody debris caused by this operation, including ditch lines and cut slopes. The contractor, at no additional cost to the Government, shall restore any damage that occurs to the roadbed or drainage structures during implementation of this contract.

Fire Precautions – All applicable state and municipal regulations must be followed and required permits obtained by the contractor. The contractor is responsible for obtaining any necessary licenses and permits, and for complying with any applicable federal, state, and municipal laws, codes, and regulations. State law requires a permit to operate power driven machinery. Permits must be obtained by the contractor from the State Forestry Office in Salem or at local offices prior to the operation of power-driven machinery.

Environmental Protection - The project work areas occur in riparian areas and aquatic habitat that have threatened species of fish and plants.

Noxious Weeds - In order to control the spread of noxious weeds, the contractor shall clean all vehicles and equipment prior to entering the work area. Cleaning shall consist of the off-site removal of soil and debris by washing with a high-pressure hose or steam cleaning, including tractor belly plates. Inspection shall be conducted at the work site and approval is required before work begins.

Work may be temporarily suspended if monitoring indicates that rainstorms have saturated soils to the extent that use of equipment could lead to road or other resource damage, or increase

BLM Medford District Office Page 16 of 79 above natural levels the amount of sediment entering the stream. This typically occurs at 25% soil moisture content.

Utilize only flagged skid roads or access routes to access stream channels.

Tracked equipment will not be permitted on bituminous surface treatment and paved surface roads.

All designated trees to remain standing shall be protected from line rub and from damage as wood and boulders are placed into the stream. All tailhold trees, block trees, and other special habitat features (such as snags and existing logs) shall also be protected from damage. Straps shall be utilized on all tailhold or block trees. Specialized cable rigging techniques (i.e., Rider blocks, directional blocking, etc.) to achieve lift or directional control during log placement shall be utilized by the contractor where appropriate to avoid damage to streambanks and other resources. Failure to utilize these techniques where feasible will be considered as operations contrary to this contract and may be a basis for terminating the contract in accordance with the "Default" clause.

Port-Orford-cedar

In projects where Port-Orford-cedar (POC) is present or could be present, sequential treatment of project units shall take place so that units which are uninfested and free of the pathogen Phytophthora lateralis (PL) will be treated first, and infested units will be treated last. Projects will be surveyed by the Government prior to treatment to determine presence or absence of PL and to determine sequence to follow for required mitigation measures defined in the POC Risk Assessment.

Access as well as egress routes, and parking locations will be determined by the COR for all projects found to be within the geographic range of POC.

Project operations in POC and or PL areas will be confined to dry season unless otherwise authorized by the COR.

When projects must occur during the rainy season (before June 15, or after October

15) or during rain events defined as when water forms puddles on the road, and access by vehicular traffic is required, the following guidelines shall be followed to prevent introduction of the pathogen to non-infested areas:

Some variation of the dates may be permitted depending on weather and soil moisture conditions, which will be defined by the COR or Project Inspector (PI). If the vehicle (and or equipment), by visual examination by the Pl, has clods of mud or organic material present, then the vehicle (and or equipment) will require washing and removal of the mud or organic material present, then the vehicle (and or equipment) will require washing and removal of the mud or organic material before entering a POC area. Entry into the area will not be allowed until this measure is followed. Prior to departure from PL area, the vehicle (and or equipment) shall be washed and free of dirt or organic material upon leaving the project area.

Cultural Resource Protection - In the event the Contractor, subcontractors, or any employees discovers any objects or sites of cultural value on the project area, such as historical or pre-historical ruins, graves, grave markers, fossils, or artifacts, the contractor shall immediately suspend all operations in the vicinity of the cultural value and shall notify the COR of the findings. Operations may resume at the discovery site upon receipt of written instructions. No objects of cultural resource value may be removed.

Hazardous Materials and Solid Waste Discovery - If, in connection with the operations under this contract, the Contractor, subcontractors, or the employees of any of them, discovers, encounters, or becomes aware of any solid waste on the project area, such as closed glass or plastic containers, barrels, household garbage, chemical and oil spills, the Contractor shall immediately suspend all operations in the vicinity of the hazardous material or solid waste and shall notify the COR of the findings. Operations may resume at the discovery site upon receipt of written instructions. No objects of hazardous materials or solid waste shall be removed from the project area.

Maintenance and Refueling - Any maintenance work and/or refueling of equipment shall occur at a minimum of 150 feet away from the stream channel and wetland areas and/or a location designated by the COR. Equipment shall be clean of dirt and grease prior to entering the stream bed. Equipment furnished shall be inspected for any leakage of petroleum products. Leakage shall be a basis for issuing an immediate shutdown of the operation. If any fuel or oil leakage occurs while working in the stream, the equipment shall immediately be removed from the stream and the Hazardous Materials Containment procedures described in 1.14 shall be followed.

Acceptable fueling areas are along access routes and will be designated on the ground by the

COR.

Hazardous Material Containment/Cleanup

The Contractor is required to develop a Spill Prevention, Control, and Countermeasure (SPCC) Plan. The SPCC Plan will be reviewed and accepted by the COR prior to initiating project work.

The SPCC plan shall, as a minimum, contain the following information:

A. Contractor Representative in charge.

B. Duties of contractor personnel.

C. Contractor emergency response procedures.

D. Contents of spill containment kit (SCK, see 3-9).

E. Spill response diagram.

Upon request, an example of an SPCC may be obtained from the District Environmental Protection Specialist.

In the event of a release as defined in Oregon Administrative Rules (OAR), Part 340, Division 108, Hazardous Waste Management, The Contractor shall immediately implement the SPCC

Plan and notify the on-site Government Representative. The Government Representative will assume control and initiate the District's Hazardous Materials Contingency Plan and Spill Containment Plan. The COR will function as Incident Commander until relieved by the District Hazardous Materials Management Coordinator (HMMC).

Public Safety - The Contractor shall exercise due caution and care when operating to prevent undue conflict with public users of roads and campgrounds. The Contractor shall post warning signs on both ends of any roads that are blocked by work in progress or when hauling boulders or logs. Signs shall be of a size, number and color which are acceptable to the Government.

DEFINITIONS

Bankfull Channel Width: The width of the stream channel measured where the stream fills the channel to the top of its banks and at a point where the water begins lo overflow onto a floodplain.

Boulder Cluster: An instream configuration made up of boulders placed strategically inside the wetted channel so as to redirect flow and reduce stream energy.

Boulder Weir: A structure made of multiple boulders wedged together which span the entire wetted and bankfull stream channel. The structure would have gaps so as to allow flow between boulders during low flow periods. Each end of the weir would tie into natural or existing rock or bedrock outcrops in the floodplain on either side of the channel to prevent scouring along the bankfull edges.

Channel Roughening: Process by which a stream channel is aggraded and made more resistant to erosion. The channel is layered with substrate and then hard-packed by jetting water into the substrate before applying the next layer. Boulders are partially buried and placed so as to reduce stream energy during high flows.

Diameter at Breast Height (DBH): Diameter of a tree at breast height, measured at a point 4-1/2 feet above ground level from the uphill side of the tree. On a cut log, DBH is determined as the diameter 4-1/2 feet above the larger end.

Hazard tree: Any tree or its parts that will fail because of a defect, and could potentially cause injury or death to people (Field Guide for Danger Tree Identification and Response, 2008)

Kev Piece (Wood): A wood piece that is no less than twice bank full stream width and has a circumference at the small end of at least 20 inches. Pieces with rootwads attached are preferred.

Large Woody Debris /LWD): A log or base of a tree, Douglas Fir or pine, that is at least 35 feet long and 14 inches DBH, and is showing little to no signs of decomposition. LWD may describe a log with a root wad attached if the log was a tree pulled over for that purpose of getting the root wad as well.

Log: For the purposes of this contract, LWD and log are used interchangeably. See LWD definition above.

Log Structure: A stable accumulation of interlocked logs and/or rootwads placed in the stream channel.

Mobilization: The movement of equipment to the initial project location specified in the task order, as well as any subsequent moves between project sites within locations specified in the same task order.

Offsite wood source: Trees or logs that will require loading and hauling by truck to the stream restoration site.

Onsite wood source: Trees or logs that do not require loading and hauling with a truck to the stream restoration site.

Road decommissioning: Treatments ranging from blocking the entrance; scarifying compacted surfaces, removing culverts; spreading slash boulders, and organic debris; and recontouring, partially or fully, the road prism.

Root Wad: The root bowl of a tree, usually a fir. In the context of this contract, root wads would only be used as part of a pullover tree, or if the contractor and COR can both agree it would be acceptable to treat the root wad and any remaining portion of the tree as a log for the purpose of transport and placement in a wood structure, though a root wad would not be required to meet the minimum 35 foot length.

Payment would be the same as for a log with the same dimensions, using the root bowl diameter to determine DBH.

Substrate: Mix of gravel and other sized material, including boulders, used to roughen a stream channel. Substrate should be composed of rounded to subangular material, and should not come from a crushed source.

CONTRACTOR-FURNISHED SERVICES AND PROPERTY

The Contractor shall always maintain an adequate work force to ensure timely completion of the work. An adequate crew includes a Supervisor and a Laborer(s).

The Contractor is responsible for the crew(s)knowing and performing the requirements of the contract.

The Contractor shall notify the Contracting Officer’s Representative (COR) in advance any changes in personnel.

Contractor shall inform the COR at all times of unit location where data is being collected.

The Contractor shall present a written performance plan, including crew size and scheduling in the prework conference.

The Contractor will keep the COR informed of crew location and when each unit is completed, or a new unit is started on a weekly basis.

The Contractor’s personnel shall have basic knowledge about performing instream restoration duties.

Crew Supervisor Requirements:

A qualified supervisor shall always be on-site directing work. Failure to maintain a supervisor who demonstrates the minimum skills and ability as shown below will result in suspension of work until an acceptable on-the-ground supervisor is provided.

A maximum of five (5) days will be allowed to produce a qualified supervisor. Failure to produce a qualified on-the-ground supervisor within five (5) days may result in the Contracting Officer issuing a non-compliance notice.

Choker/Setter/Sawyer and Tree Climber shall be certified and/or provide sufficient documentation as to their qualifications. Cutters shall be equipped to cut trees up to 64 inches in diameter (DBH). They shall be experienced with directional falling and use of jacks and experienced working with and around heavy equipment and use of chokers. In addition, the Tree Climber shall be a certified tree climber with experience in tree pulling operations and capable of climbing trees up to 64 inches in diameter.

Certified Flaggers shall be required to control traffic flow where project operations may be a safety concern to the public on Federal, State, Municipal, and Private Roads. Flagging operations shall meet standards set by ODOT for all road ownerships.

Laborers shall be capable of performing the following task: 1) packing and hauling rigging gear to complete instream structures, 2) seeding and mulching areas of disturbance and 3) assisting equipment operator with a variety of on-the-ground operations and other laborer type activities.

Spill Containment Kit - The Contractor is required to have a Spill Containment Kit (SCK), a described in the SPCC on-site during any operation and provide training to employees on how components of the SCK are used, The SCK must be designed for use with petroleum products, and must contain, as a minimum, the following items:

A. Four Bales (8 Booms/Bale, of 8 inches x 10 feet Absorbent Booms).

B. Two Bales (100 Pads/Bale, of Absorbent Pads, minimum of 17 inches x 19 inches x ¼ inches).

C. One Absorbent Sweep (minimum of 18 inches x 100 feet x 3/8 inches).

D. Two 30–50-gallon garbage cans.

Equipment - The Contractor shall provide all machinery necessary to successfully complete tasks within this contract while protecting the environment. The Contractor shall provide the qualified operator(s) and operating supplies such as fuel, oil, and grease. The Government reserves the right to require the Contractor, in writing, to remove from the project any operator, who in the opinion of the COR, is found to be inexperienced or operates the equipment in an unsafe manner.

The Contractor shall provide a replacement operator within 24 hours.

Equipment Condition and Maintenance - Equipment furnished shall be in acceptable working condition and shall be subject to Government pre-use inspection. The Government reserves the right to reject equipment which is not in safe and operative condition. The Contractor and the operator shall perform all necessary repairs and servicing to maintain the equipment in good operating condition during the period of use. All repairs and/or replacement equipment shall be made within 48 hours of breakdowns.

Operating Permits - State Law requires a permit to operate power driven machinery. The Contractor shall obtain permits from Oregon State Department of Forestry prior to the operation of power-driven machinery. This requirement applies to tractors, chainsaws, and brush cutters.

Documentation of permits will be furnished to the government before the start date of the task order.

Straw mulch and seed shall be applied to exposed earth on decommissioned routes, access routes, landings, log drag routes, and areas of disturbance as designated by the COR with certified weed free straw and seed mixture as directed and approved by the COR.

GOVERNMENT-PROVIDED PROPERTY

The Government will furnish to the Contractor the following materials, supplies, property or services at the pre-work conference or at the time a task order is issued the following:

A. All location and project maps and typical drawings.

B. Gate keys.

C. Seed mix and straw will be provided by the BLM.

SPECIFIC TASKS

Some or all of the following tasks may be required for any given project. General Excavation and Haul Work are time and equipment items and are only meant for and will only apply to work not covered by another sub-item.

Mobilization - per piece of equipment

Mobilization includes the movement of each piece of equipment listed as sub-item 1C, 17A, 17B, or 17C that is transported to the ordering office. Subsequent moves from the ordering office to the project site as well as any subsequent moves between project sites specified in the same task order will be paid mileage rates as identified in sub-items 1A-1C. The number of mobilizations per piece of equipment will be specified in the task order. Access to all sites will

BLM Medford District Office Page 22 of 79 be by existing paved, gravel, and natural surface roads. Additional service vehicles needed to support, service, or maintain equipment during fire season and non-fire season, including dump trucks, shall be included in the mobilization cost of mobilization of subitems 1C and 17A-l 7C.

Cost of additional mobilizations not requested by the government, such as movement of equipment between sites for the convenience of the contractor, or to deal with broken equipment, will be incurred by the contractor.

In order to control the spread of noxious weeds, the contractor shall clean all vehicles and equipment prior to entering the work area. Cleaning shall consist of the removal of soil and debris by washing with a high-pressure hose or steam cleaning, including tractor belly plates.

Inspection shall be conducted at the work site and approval is required before work begins.

Falling Snags and Trees

Each task order will identify individual snags and trees to be felled and ordered by levels as specified in Section B - Schedule of Items. They may be felled directly into streams, decked and hauled to various instream sites or stock piled for later use. The snags and trees may be located along roadsides or off-road. The contractor shall only cut trees specified by the COR, i.e. trees and snags that are identified with maps, flagging and/or paint. Trees shall be felled in a manner that minimizes damage to adjacent trees. Trees and snags shall be felled in a manner to achieve maximum length. If trees are determined to be unsafe to cut by the Contractor, the Government may substitute an alternate tree with no adjustment in price. The Contractor shall buck each piece to designated lengths of logs. Trees designated for in-stream structures shall be cut to no less than 35 feet.

Adjacent trees that are severely damaged (more than 50% circumference of cambium exposed) will be paid for by the contractor (market value).

For line attached falling: tree climbers shall attach lines to trees from equipment that allows equipment to assist sawyers in directional falling to minimize damage to roads and to avoid archeological sites, buildings, etc.

Pulling Over Trees with Rootwads Attached

Each task order will identify individual trees to be pulled over and ordered by levels as specified in Section B - Schedule of items. They may be pulled directly into streams, decked and hauled to various instream sites or stock piled for later use. The trees may be located along roadsides or off-road. Contractor shall only pull trees specified by the COR i.e. trees and snags that are identified with maps, flagging and/or paint. Trees shall be pulled in a manner that minimizes damage to adjacent trees. Trees shall be pulled in a manner to achieve maximum length. If trees are determined to be unsafe to pull by the Contractor, the Government may substitute an alternate tree with no adjustment in price. Trees shall be pulled over in a manner that will achieve maximum length with the root system attached. Trees designated for instream structures shall be cut to no less than 35 feet.

Equipment may not enter stream channels in order to pull over streamside trees. Tree climbing

BLM Medford District Office Page 23 of 79 will generally be needed for tree pulling activities. Parts of the root system may need to be sawed off and the soil saturated with a pump system to facilitate tree pulling. If a tree breaks without the root wad attached, the pay item for Sub-item 2 shall apply for that tree. All trees used for tailholds shall not be damaged in any manner.

Adjacent trees that are severely damaged (more than 50% circumference of cambium exposed over half the tree length) will be paid for by the contractor (market value).

Skidding and Yarding Logs to Landing

Individual task orders will specify levels based on site specific information. Individual task orders will specify ground-based equipment or cable system required to yard logs to identified roadside staging area. Retrieval distances range from 1 to 1,000 feet. Limbing and bucking logs may be necessary prior to moving; log lengths shall be no less than 35 feet. Retrieval of wood pieces shall be completed without compacting or exposing soil to 5 percent or less of the project area. Cable yarding from the road will be required in areas where the slope is over 30 percent or when other resource values exist. Disturbed areas shall be seeded and mulched.

Load, Haul, and Unload Logs

The Contractor shall provide all necessary equipment needed to load, haul and unload regular and oversized wood pieces. Logs shall be loaded and hauled from designated cutting areas to designated project areas and unloaded at designated staging area(s) or individual sites along roads. Hauling distances will vary from 1 mile to 70 miles. Payment is based on one-way hauling distance. Log retrieval and placement sites may include several locations within a project area.

Wood pieces will range in size from l 4 inches up to 64 inches at the large end and 35 feet up to 80 feet in length and may have root wads and branches attached. Log hauling resulting in resource damage such as damaging standing trees will be paid for at the contractors' expense.

Roads used under this contract shall be restored to their original condition at the contractor's expense. All roadways shall be cleared of woody debris caused by this operation, including ditch lines and cut slopes. The contractor, at no additional cost to the Government, shall restore any damage that occurs to the roadbed or drainage structures during implementation of this contract.

The contractor is responsible for any flaggers, signage, and state or federal permits needed for hauling on county, state, or federal roads and highways and bridges.

Cable Yard Logs and Trees into Streams

This item would require trees and logs to be placed instream without entering the stream channel and from distances up to 1,200 feet away from the stream channel. Upon completion, trees and logs will be intertwined with standing riparian trees and interlocking with other instream structures (boulders and wood pieces). Yarding and positioning of wood in streams shall be completed with minimal ground and vegetative disturbance. All standing trees not designated for removal shall be protected from line rub and from damage as wood is placed in the stream. Any damage to occur to standing trees shall be paid for at the contractor's expense. Trees that are

BLM Medford District Office Page 24 of 79 severely damaged (more than 50% circumference of cambium exposed) will be paid for by the contractor (market value). Equipment shall be capable of positioning trees in the stream from up to 1,200 feet away from the channel.

General Excavation and Haul Work

Equipment such as excavators, bulldozers, front- end loaders, 10-yard dump trucks, or any combination of these pieces of equipment along with operator(s) shall accomplish a variety of stream and terrestrial restoration objectives under this item. Work may consist of removing culverts, crossdrains, legacy structures removal and other non-functional instream structures.

Streams may be excavated to create pool or jump pool habitat, install grade control structures, or otherwise restore channel features to functional condition. Excavated materials may be used to armor neighboring structures or may be required to be placed away from stream channels as indicated by the COR. The equipment may also be used in some areas to create off channel rearing habitats.

Other work under this item may consist of obliterating or decommissioning roads or modifying other features as necessary to reduce sources of sediment. This would also include scattering onsite slash, logs, rocks and other organic debris on disturbed surfaces. Various types and sizes of equipment based on weight shall be utilized depending on nature of work and site conditions.

The nature of the equipment used maybe determined by the contractor, as long as the COR approves, it fits into the appropriate weight categories, and will accurately and efficiently meet objectives.

The contractor shall be responsible for submitting a daily equipment time report to the COR.

Any submitted time report will contain the name of the project, relevant task item or number, and time reported to the nearest 15 minutes.

Some site conditions may require the contractor to dewater streams prior to road decommissioning (see Dewatering)).

Dewatering

Where instream work requires stream dewatering, such as culvert removal, the contractor shall convey stream flow not to exceed two cubic feet/second (or 898 gallons/minute) around the construction site and discharge the flow back into the stream at a stable location below the site.

The contractor shall also contain any turbid water originating from the construction site and convey that flow to a stable location and discharge it so it does not re-enter the stream. Prior to commencing work, the contractor shall provide a dewatering plan to the COR for approval. The plan will address how to protect fish and prevent the release of above-normal turbidity into downstream waters. The contractor is required to monitor and comply with acceptable turbidity levels as mandated by State of Oregon regulations.

The contractor may need to make use of sumps, pumps, and settling ponds.

Soil Stabilization

Straw mulch and seed shall be applied to exposed earth on decommissioned routes, access routes, landings, log drag routes, and areas of disturbance as with certified weed free straw and seed mix. The seed mix and straw shall be provided to the purchaser from the BLM. The contractor may be responsible for transporting furnished straw from a government designated site to the project site, as identified during the pre-work conference. Seeding shall occur prior to mulching with a native seed mix at a rate of 20 lbs/acre.

QUALITY CONTROL

The Contractor shall propose and adopt a quality control inspection system to ensure that the work is progressing in compliance with specifications. This quality control system shall be addressed in the Contractor's proposal. See Section L, Instructions for Preparation of Technical Proposal. It is the responsibility of the Contractor to perform inspections of his/her work, including the work of Sub-Contractors

A copy of the complete contract, including specifications, maps, exhibits, fire procedures, modifications, etc., shall be kept on the job site at all times for the Contractor's foreman and crew to refer to for compliance. Government inspection constitutes quality assurance and does not substitute for the Contractor's quality control. Written reports of the Contractor's quality control inspections shall be available for periodic review by the CO, COR or Pl upon request.

Contractor's records shall include: (l) project names (2) treatments (3) work dates (4) supervisor/inspector name and (5) supervisor's signature and date.

SURVEILLANCE, INSPECTIONS, ACCEPTANCE AND PAYMENTS

All work performed by the Contractor shall be subject to periodic inspections as a basis for payments and recommendations for adjustments in work quality during the performance of each item and/or when units are completed to provide adequate and timely quality assurance.

Acceptance of work is based solely on Government inspections and documentation. Government acceptance will be determined based on compliance with the contract requirements

Payment will be based on the Government's inspection results. The Contractor's inspection results are to be used as a guide for the Contractor's use in complying with contract specifications and not as a basis for payment

Payment will be made as project areas are inspected and accepted. Written notice of completed units shall be submitted (by fax) to the COR after completion of treatments and prior to invoicing for payment
