AMENDMENT OF SOLICITATION/MODIFICATION OF CONTRACT CONTINUATION

1. Page 3, Output #5, Institution-Based Physician Services. General Practitioner, Optometry, Audiology, and Pathologist/Lab Director shall be paid upon the provision of a defined session. A session is equal to one (1) hour.

For session priced items, sessions which exceed the established session or are less than a full session shall be prorated to the nearest quarter hour.

The estimated quantities for the General Practitioner (3-4) and Pathologist/Lab Director are changed to the following:

General Practitioner (3-4): Session Duration – 8 hours (Approximate) Session Quantity – 6240 sessions per year

Pathologist/Lab Director: Session Duration – 8 hours (Approximate) Session Quantity – 288 per contract year

2. Page 4, Output #6, Outpatient Institution-Based Physician Services and Invasive/Surgical Procedures: The introductory language is deleted and replaced with the following:

Quantity estimates are for the base period and each option. Estimated quantities shall remain constant for the base year and all options. The duration of visits and procedures are listed for informational purposes only as contracted rates will be based upon the Medicare allowable for the clinic and procedure performed. Oral Surgery Clinic/Procedures and Anesthesiology shall be paid upon the provision of a defined session. A session is equal to one (1) hour. For session priced items, sessions which exceed the established session or are less than a full session shall be prorated to the nearest quarter hour.

The estimated quantities for the Oral Surgeon are changed to the following:

Oral Surgery Clinic/Procedures: Session Duration – 8 hours (Approximate) Session Quantity – 12 sessions per year

The following language is added: The Contractor is responsible for the issuance of dosimetry badges to the appropriate individuals on their staffing plan on a monthly and/or quarterly. Contractor is responsible for all cost for acquiring dosimetry badges.

10. Page 22. Output #6. The following statement is added to Output #6: The offeror’s technical proposal shall discuss in detail the diversity of services that it is capable of providing to accomplish Output #6, including a proposed plan for securing substitute services in the event of staff absences.

11. Page 23. Within Output #7 – Wound Care Physician (Plastic Surgery), the reference to Output #6 on page 23 is changed to Output #7. Additionally, the Title Wound Care Physician (Plastic Surgery) is deleted and replaced with “Wound Care Treatment Specialist”

12. Page 24, Output #9, Invoice Definitions. The reference line item for the following services are changed:
