About this file

This document is a Bidder Information and Certification Sheet for Request for Quotation (RFQ) #S-16500-00014294, issued by a state agency in Oregon, pertaining to support and maintenance services for one Universal Power Supply. The RFQ requires bidders to complete a comprehensive certification form that details their business information, compliance with state regulations, and commitment to non-discriminatory practices. The bid is valid for 90 days, and the selected contractor will be expected to perform services according to the scope of work defined in the final negotiated contract.

The certification sheet includes critical vendor requirements such as tax law compliance, non-discrimination policies, workplace equity standards, and provisions related to employee wage discussions. Notably, the RFQ stipulates that for contracts anticipated to exceed $150,000, vendors must have a written policy preventing sexual harassment and discrimination. Vendors must also certify they are not on federal exclusion lists, have no conflicts of interest, and have prepared their bid independently. The document emphasizes compliance with Oregon Revised Statutes (ORS), including regulations around business enterprise diversity, wage equity, and potential penalties for false claims.
