About this file

This is a Reference Check Form attachment for RFP #S-63500-00015974 issued by the State of Oregon's Department of Administrative Services (DAS) Procurement Services for Telemetry Helicopter Flights for Inventory and Capture of Gray Wolves Throughout Eastern Oregon. Proposers are required to submit three completed Reference Check Forms from professional references, with at least two references being non-Oregon Department of Fish and Wildlife (ODFW) current professionals and one from either a different firm/client or from ODFW. References must be from different firms and clients and should demonstrate the proposer's ability to comply with project requirements, which involve providing a helicopter, experienced pilot, experienced wolf capture crew, fuel truck, and driver to support capture and monitoring operations in Eastern Oregon.

The Reference Check Form evaluation is worth a maximum of 20 points, with each reference assessed on a scale of 0 to 5 points across seven evaluation criteria: overall quality of services and relationship, safety performance, customer service responsiveness, pilot and crew communication and flexibility during operations, pilot skills and experience in wildlife capture in remote locations, gunner experience and ability in wildlife capture in remote locations, and likelihood of future contractor engagement. The Agency will make three attempts to contact each reference, with proposers receiving a score of zero for any reference that cannot be successfully contacted. References are rated on a satisfaction scale where 0 equals not satisfied, 2.5 equals moderately satisfied, and 5 equals extremely satisfied.
