Table 4.3-1: Operational Staff Incident Response Times.

DOT-RFP-23-6179SD Exhibit A Scope of Services Page 10 of 17

Incident responses by SWZ operational staff shall vary depending on incident classification. Incidents classified as Major, shall be reported to emergency services within the allotted maximum response time. The Consultant shall submit an incident response plan, as part of the Concept of Operations, for approval by the Department. The Incident Response Plan shall, at a minimum, show step-by-step the actions of operational staff following incidents of multiple severities, assigning a hierarchal contact list for incident reporting. Failure to follow the incident response requirements and procedures outlined in this document and in the Department approved Concept of Operations shall result in financial consequences outlined in Section 6.1.

The Consultant shall also be responsible for providing detailed reports for all incidents involving the SWZ system.

Incidents include physical interactions between the SWZ deployment(s), vehicles, and other entities (e.g., pedestrians, bicyclists, animals, buildings, infrastructure, landscape, any other tangible property, that results in personal injury or physical damage of any magnitude). Reports shall be submitted monthly to FDOT District 6, at least two (2) days before the monthly progress meeting. Incidents requiring the attendance of emergency services shall require an incident report be submitted three (3) days from time of incident and may require additional documentation at the request of the Department. Reports shall include the following minimum information:

Date and time of incident Location of incident (latitude and longitude within 1-meter) General description of the event Description of property damage Smart Work Zone system equipment involved Name individuals contacted to report incident, along with time and date of reporting Detail all injuries reported, their severity, and if emergency services were required Ambient environment conditions, including, but not limited to, lighting, weather, etc.

4.3.2 SWZ Performance Requirements

The Consultant shall be responsible for providing operation of the SWZ system 24 hours a day, 7 days a week, including available SWZ operational staff, deployed and functional field equipment, and a fully operational SWZ control center. The SWZ shall be considered fully operational and available when the SWZ equipment and systems meet the requirements outlined in this document and the objectives detailed in the Smart Work Zone US 27 / SR 25 (Okeechobee Road) Operational Report. The minimum acceptable availability of the fully operational SWZ system shall be 98% of the expected operational uptime. Failure to meet the minimum availability rate shall result in financial consequences outlined in Section 6.1.

4.3.2.1 System Availability:

SWZ system performance shall be evaluated by the availability of deployed field devices and their ability to meet the system requirements of this Agreement. The quantity of deployed SWZ field devices and sensors may change depending on MOT phasing and may vary at times during a given month or pay period. Therefore, SWZ system availability shall be based upon a weighted-average of the amount of time field devices and sensors are deployed and operational. The operational status or availability of field devices shall be calculated by device health monitoring software to be provided by the Consultant.

The device health monitoring software shall calculate SWZ system availability as a percentage of the average amount of time deployed field devices and sensors are operational over their expected operational uptime in a given month. For example, the expected operational uptime of the entire SWZ system shall be 24 hours a day, 7 days a week, however the expected operational uptime of a field device or sensor is 24 hours a day, 7 days a week, for the duration of its expected deployment.

SWZ system availability may also be assessed by the field device deployment’s ability to satisfy requirements detailed in this Agreement. In the event a field deployment does not satisfy all requirements agreed upon in this contract, the Department shall provide a written assessment of the situation and document the missing devices required to provide a satisfactory field deployment. The Department reserves the right to audit field device deployments at any time, including retroactive audit based upon the monthly Performance Report. The missing devices identified by the Department, shall be included in the SWZ system availability calculation, and considered

DOT-RFP-23-6179SD Exhibit A Scope of Services Page 11 of 17 un-operational for the entire duration of their expected operational uptime. For example, if a deployed cctv camera is unable to provide images of vehicles entering the work zone (a requirement from Section 4.2.1 of this Scope), the Department may determine the deployment is missing required surveillance coverage of the work zone and is therefore not satisfying the requirements of this Agreement. As a result, that missing device shall be considered un-operational and shall be considered as such from the time the malfunction was first observed to the end of the MOT phase or until the missing device is deployed, whichever comes first.

4.3.2.2 System Performance Software:

The SWZ system must have a backend software capable of monitoring the operational status or health of all field devices and sensors providing data back to the SWZ control center.
