About this file

This is a bid sheet attachment (Attachment D) for RFP27-011 issued by the Washington Military Department for stormwater infrastructure inspections, cleaning, and repair services at Camp Murray and the Seattle Readiness Center. The contract will have an initial 12-month term with four optional 12-month renewal periods. The bid sheet requires bidders to complete a table identifying all employee positions that will work on the project, establish hourly rates for each position that include all costs such as travel, and estimate the expected percentage of hours each employee type will contribute to the project, with the total percentages equaling 100%.

Pricing will be evaluated using a weighted rate calculation that compares each bidder's weighted average hourly rate against the lowest bidder's weighted rate, with the scoring formula presented as (Lowest Bidder's Weighted Rate ÷ Bidder's Weighted Rate) × 100. Bidders must submit completed bid sheets to the Procurement Coordinator at contracts.office@mil.wa.gov. The bid sheet format includes example categories such as Project Manager, Senior Engineer, Technician, and Admin positions but requires bidders to identify the actual employee titles and associated labor costs relevant to their proposed service delivery approach.
