SCOPE OF WORK:

The Contractor is requested to provide a variety of non-personal medical professional services for (20) different specialties for a 12 month base period and four (4) 12-month option periods. The intent of this contract is to establish a method whereby the Uintah and Ouray Health Center (U&O) can minimize disruptions in clinical service because of staff changes and shortages over the next 5 years. The specific need for each of these specialties is difficult to predict and therefore it is likely that U&O may not use all the specialties each year, however having them available when the need arises will minimize service disruptions. Also, even when the best plans are laid to hire and retain staff, our remote location makes this difficult and we can have staff shortages despite trying to hire a full-time employee. Requests for contractor coverage will be based on the consistently changing needs of the clinic.

All services shall be performed at the following location:

Uintah and Ouray Health Center
