About this file
This document is an RFP Submission Checklist designed by the City of Philadelphia to guide applicants in completing accurate and timely proposals for Professional Services RFP opportunities at or above the formal RFP threshold. The checklist addresses procedural compliance requirements, including submission before stated deadlines through the eContractPhilly electronic portal, verification of proper opportunity number selection, and confirmation that proposals are signed and submitted by authorized signatories. Applicants must demonstrate how their proposals meet the requirements outlined in the Department's Scope of Work section, include a compliant Cost Proposal, and adhere to all format and content requirements specified in the RFP's Proposal Format, Content, and Submission Requirements section.
The checklist requires applicants to review the entire RFP and Contract Attachments, including Appendix A General Provisions, and to explicitly state any objections to these provisions in their proposals, as objections are not favored and may be weighed against the application. Financial documentation must include a Statement of Financial Capacity along with audited or unaudited financial statements, disclosure of any bankruptcy filings within the past five years, or the most recent IRS Form 990 for non-profit organizations. All applicants must complete Appendix C: City of Philadelphia Tax and Regulatory Status and Clearance Statement, disclose information required under Chapter 17-1400 of the Philadelphia Code regarding local political contributions, and verify that the name and Tax Identification Number registered on eContractPhilly match those on the application and W-9 form. Optional submissions include Appendix D: Local Business Entity or Local Impact Certification and Appendix E: LGBTQ Applicant Opportunity Data.
