About this file
This is an RFP Submission Checklist for Professional Services opportunities in Philadelphia, Pennsylvania. The checklist serves as a comprehensive tool to ensure applicants submit complete, accurate, and timely proposals meeting the City of Philadelphia's formal RFP threshold requirements. The document outlines essential submission components including confirmation of timely submission before stated deadlines, detailed explanations of how applicants meet departmental requirements outlined in the Scope of Work, and inclusion of cost proposals consistent with specified requirements. Proposals must adhere to format and content standards detailed in the RFP's Proposal Format, Content, and Submission Requirements section, and applicants are required to review all RFP documents and Contract Attachments, including Appendix A General Provisions, disclosing any objections within their proposals.
The checklist mandates submission of Appendix C: City of Philadelphia Tax and Regulatory Status and Clearance Statement and a Statement of Financial Capacity accompanied by audited or unaudited financial statements, bankruptcy filing disclosures from the past five years, or IRS Form 990 for nonprofit organizations. All applicants must disclose required information under Philadelphia Code Chapter 17-1400, including local political contributions through their eContractPhilly accounts. Optional submissions include Appendix D for Local Business Entity or Local Impact Certification and Appendix E for LGBTQ Applicant Opportunity Data. All proposals must be submitted electronically through eContractPhilly under the correct opportunity number, signed by an authorized signatory using the platform's sign and submit function, with the applicant's registered name and Tax Identification Number matching across all submission documents including Appendix C and the W-9 form. The checklist was revised in July 2025.
