About this file
This is an RFP Submission Checklist issued by the City of Philadelphia for Professional Services opportunities at or above the formal RFP threshold. The checklist serves as a comprehensive tool to ensure applicants submit complete, accurate, and timely proposals through the eContractPhilly platform. Key submission requirements include demonstrating how the applicant meets the Department's Scope of Work requirements, submitting a cost proposal consistent with RFP specifications, and adhering to format and content requirements outlined in Section III of the RFP. Applicants must review all RFP attachments, including Appendix A General Provisions, and formally state any objections within their proposal submission.
Required documentation includes the Office of Economic Opportunity Solicitation for Participation and Commitment Form, Appendix C Tax and Regulatory Status and Clearance Statement, and a Statement of Financial Capacity supported by audited or unaudited financial statements, bankruptcy disclosures, or IRS Form 990 for nonprofits. Applicants must disclose all information required under Philadelphia Code Chapter 17-1400, including local political contributions, lobbyists, and subcontractors. The registered business name and Tax Identification Number on eContractPhilly must match the name and TIN on the application, Appendix C, and the W-9 form. Optional submissions include Appendix D for Local Business Entity or Local Impact Certification and Appendix E for LGBTQ Applicant Opportunity Data. Technical assistance is available through the eContractPhilly website, Chapter 17-1400 regulations, email at econtractphilly@phila.gov, or by calling 215-686-4914.
