Summary of RFP PS-26-0006 Fee Schedule - Occupational Physical Therapy Services
This is a Fee Schedule exhibit for the City of Phoenix's Request for Proposals (RFP PS-26-0006) for Occupational Physical Therapy Services, a five-year contract commencing on or about July 1, 2026. The City seeks comprehensive occupational physical therapy support for its municipal workforce of over 16,000 employees across a minimum of 35 locations throughout the Phoenix Metropolitan area in Maricopa County. Service requirements include immediate electronic patient status reporting, initial evaluation scheduling within one business day, and a network of licensed therapists providing extended hours Monday through Saturday with evening availability. The non-mandatory pre-offer conference is scheduled for September 8, 2025, at 10:00 a.m., with written inquiries due by September 15, 2025, and proposals due October 1, 2025, at 2:00 p.m. The contract will commence upon City Council approval and execution by the City Clerk's department.
The fee schedule accounts for 350 of the 1,000 total evaluation points (35% of the scoring criteria). Contractors must submit their pricing as part of Section 3.2 of the Vendor Questionnaire, Question 7. The RFP includes a "Most Favored Nations" clause requiring contractors to provide the City with more favorable pricing terms if they have established more beneficial fee schedules with other clients having similarly sized and risked employee pools; the City may elect to adopt such more favorable terms within 30 days of notification. Historical spending on physical therapy services for workers' compensation totaled approximately $11.5 million over 2022-2024, indicating substantial contract value. Performance guarantees include deductions such as $200 reductions for missed electronic transmission deadlines and additional discounts for excessive wait times or delayed utilization reviews. Contractors must demonstrate a minimum of five years of experience administering occupational physical therapy services and provide comprehensive insurance coverage, including commercial general liability, automobile liability, workers' compensation, professional liability, and network security and privacy liability.
