About this file

This is an RFP forms document for the City of Watertown, Wisconsin's Department of Wastewater, soliciting proposals from qualified vendors for a three-year contract (2021-2023) for hauling and land application of cake biosolids with an option to extend for an additional two years. The contract covers annual land application of approximately 2,250 cubic yards of biosolids. Vendors must submit complete proposals including a signed Signature Affidavit (Form A), Receipt of Forms and Submittal Checklist (Form B), Vendor Profile/Qualifications (Form C), References (Form D), and Fee Proposal in a separate sealed envelope (Form F). The RFP document includes provisions for local vendor preference, granting scoring advantages to suppliers within City of Watertown limits as of the bid due date. Proposers must certify that they have not engaged in collusion, that proposals are independently developed, and that all submissions are truthful under penalty of perjury.

The fee proposal requires vendors to submit a lump sum cost on a per-cubic-yard basis for annual land application services, with all direct and indirect necessary expenses included. Vendors must indicate acceptance of credit card payments with no additional fees, payment via Electronic Funds Transfer (EFT), and whether they will allow piggybacking of the contract to other government agencies. The City of Watertown requires contractors to maintain comprehensive insurance coverage prior to commencing work, including Worker's Compensation insurance with statutory limits, Commercial General Liability and Automobile Liability insurance with minimum limits of $1,000,000 per occurrence, and Umbrella Liability coverage of $5,000,000. The City must be named as an additional insured on all policies, and contractors are responsible for ensuring all subcontractors meet the same insurance requirements. Any additional services beyond those described in the agreement require written authorization from the City's designated representative, with hourly rates for various staff positions to be provided separately.
