About this file

This document is an Attachment C - Proposer Information and Certification Sheet for RFP S-10700-00005076 - Abandoned Derelict Vessel and Structure Removal, issued by the State of Oregon. The RFP seeks proposals for Master Service Agreements to provide services for the removal of abandoned derelict vessels and structures within the jurisdictions of Lincoln County and the City of Newberg. Proposers must certify their understanding and acceptance of the RFP requirements, including the Master Services Agreement terms and conditions. Proposers must also provide information about their organization, such as legal name, address, entity type, and contact details. Additionally, the certification sheet covers proposer representations regarding payment of taxes, non-discrimination, and conflict of interest, among other requirements.

The RFP does not provide specific details about pricing, set-asides, incumbents, funding sources, or contract amounts. However, it does require proposers to certify that their proposal is a firm offer for 180 calendar days following the opening, and that if awarded a Master Services Agreement, the proposer agrees to perform the scope of work and meet the performance standards set forth in the agreement.
