About this file

This is an RFP Amendment document for Lab Sample Containers issued by the State of Michigan (EGLE) under RFP No. 210000001807. The procurement requests lab sample containers with specific preservative labeling requirements. The RFP underwent three timeline amendments between May 17 and May 18, 2021. Under the final updated timeline (as of May 18, 2021), the sample deadline was set for June 1, 2021 at 5:00 p.m. Eastern, the deadline for bidder questions was June 7, 2021 at 3:00 p.m. Eastern, anticipated answers to questions were to be posted June 8, 2021 at 3:00 p.m. Eastern, and the proposal deadline was June 22, 2021 at 3:00 p.m. Eastern. The anticipated contract begin date remained October 1, 2021. Contractors must provide pricing for all bottle types they can supply from an attached pricing catalog spreadsheet, leaving blank any items they cannot provide. All bottles must be labeled with preservatives completely spelled out, including the amount and concentration of preservative.

The amendment clarifies product specification requirements, specifically requiring that preservative concentration be included on bottle labels. The acceptance and inspection process involves EGLE inspecting lot numbers on incoming shipments. Shipments with previously accepted lot numbers are automatically accepted, while new lot numbers undergo quality control testing with reagent grade water to ensure no contamination interferes with analysis. The updated acceptance procedures specify that if a lot check fails, EGLE will notify the Contractor, arrange pickup of rejected bottles at the Contractor's cost, and expect delivery of replacement bottles within five business days. The document contains no information regarding pricing terms, set-asides, incumbent contractors, funding sources, or contract duration.

ORIGINAL ACCEPTANCE

6. Acceptance

6.1. Acceptance, Inspection and Testing

EGLE will inspect the lot number of each shipment. If the lot number is the same as a previous shipment, bottles are accepted. If the shipment contains a new lot number EGLE will run a lot check (this entails a quality control check of the sample bottle filled with reagent grade water. The parameters of interest are tested to ensure there is no contamination in the container that will interfere with the analysis). If the lot check passes, bottles are accepted. If the lot check does not pass, the bottles will be sent back to the manufacturer at the Contractor’s cost.
