About this file
This document is an Attachment F - Certified Disadvantaged Business Outreach Plan for RFP 5732 - Pharmacy Cost to Dispense Survey, issued by the state of Oregon. The RFP seeks proposals from qualified firms to provide a pharmacy cost to dispense survey for the state of Oregon, including the jurisdictions of Marion County and the cities of Salem. The RFP requires proposers to detail their steps to solicit certified minority-owned, woman-owned, service-disabled veteran-owned, and emerging small businesses for subcontracting opportunities if awarded the contract. Proposers must indicate if they are a certified firm, provide a list of certified firms and other minority/woman/veteran/small businesses they have contracted with in the past two years, and describe any foreseen subcontracting opportunities and their willingness to report subcontractor identities and values to the state's certification office.
The RFP does not provide specific pricing terms, set-asides, or funding information. Proposers must simply outline their outreach plan for certified disadvantaged businesses as part of their proposal submission.
