About this file

This is a USDA Good Agricultural Practices and Good Handling Practices (GAP & GHP) Audit Verification Checklist form used by the United States Department of Agriculture, Agricultural Marketing Services, Specialty Crops Inspection Division to assess fresh produce operations in Wisconsin. The audit evaluates participant efforts to minimize microbial contamination risks for fresh fruits, vegetables, nuts, and miscellaneous commodities based on FDA guidance and recognized good agricultural practices. The comprehensive checklist is designed to be conducted at produce facilities and farms, with auditors documenting findings across seven major evaluation elements: General Questions, Part 1 (Farm Review, 190 possible points), Part 2 (Field Harvesting and Field Packing Activities, 185 possible points), Part 3 (House Packing Facility, 290 possible points), Part 4 (Storage and Transportation, 255 possible points), Part 6 (Wholesale Distribution Center/Terminal Warehouses, 410 possible points), and Part 7 (Preventive Food Defense Procedures, 180 possible points). The audit may be announced or unannounced and can be requested as a USDA Commodity Procurement Audit.

The audit utilizes a pass/fail scoring system requiring facilities to achieve a minimum passing score of 80% of adjusted possible points with no automatic unsatisfactory conditions identified. Automatic unsatisfactory conditions include evidence of rodents or excessive pests in produce, employee hygienic practices jeopardizing food safety, produce contamination by blood or bodily fluids, falsification of records, or failure to answer affirmatively on critical food safety program questions. The checklist requires documentation through policies, standard operating procedures, records, and supporting materials to demonstrate conformance across sanitation, hygiene, water quality, pest control, temperature control, traceability, food defense procedures, and worker health practices. Auditors complete detailed reports noting non-conformities with required corrective action plans and implementation timelines for any deficiencies identified during the inspection.
