About this file
Addendum #1 to Request for Proposal DOT-RFP-26-6199LS, issued by the Florida Department of Transportation District Six, clarifies submittal requirements and answers technical questions regarding a solicitation for Cultural Resource Assessment Surveys and NEPA Compliance Services in Miami-Dade and Monroe Counties. The addendum confirms that proposers must submit one electronic copy via email to D6.Purch@dot.state.fl.us and one physical copy on CD to the District Office at 1000 NW 111th Avenue, Miami, FL 33172. The proposal due date remains November 25, 2025, at 10:00 AM. Key clarifications address project manager qualifications, personnel listing requirements, references, and project history documentation within the prescribed 20-page limit. The addendum specifies that the project manager must possess Florida-based PD&E or NEPA study experience rather than out-of-state equivalents, and that references and project lists must be provided for each key staff member assigned to the contract rather than for the firm as a whole.
The addendum further clarifies that Vendor Certification Regarding Scrutinized Companies List forms and E-Verify documentation must be submitted as part of the proposal and that the RFP page 10, Section 21.2, has been revised to address submittal criteria changes. All other contract documents remain unchanged from the original solicitation. The addendum does not contain pricing information or cost proposals, as those are addressed separately in the main RFP. Vendors must comply with MyFloridaMarketPlace registration, maintain liability insurance with combined bodily injury limits of at least $200,000 per person and $300,000 per occurrence, and carry property damage insurance of $200,000 per occurrence. Subcontractors and specialized consultants require pre-approval from the District Cultural Resources Coordinator.
