About this file
This is a pricing schedule submission form for Request for Proposal No. 260000001604 issued by the Michigan Truck Safety Commission (MTSC) for a Commercial Motor Vehicle (CMV) Safety Training and Education Program. The proposal requires bidders to provide comprehensive CMV safety training and education programs to a substantial number of individuals throughout Michigan. Contractors must deliver training courses, seminars, and outreach activities supported by physical materials including printed guides and educational resources. The pricing schedule must be submitted in modifiable format (Microsoft Word or Excel), with an optional non-modifiable PDF version. Failure to complete the schedule as requested may result in proposal disqualification.
The pricing structure is based on a per-person cost model, with bidders required to specify the maximum number of individuals they can train per fiscal year (October 1 through September 30), the cost per individual, and total costs for both one year and the five-year contract term. Pricing must include all costs such as one-time setup charges, travel expenses, office supplies, printing, and educational materials, as the State will not reimburse separate line-item expenses. Bidders are encouraged to offer quick payment discount terms. Monthly invoices for training and outreach services will be submitted to the Office of Highway Safety Planning (OHSP) for reimbursement based on individuals reached, while printing cost invoices will be submitted for full reimbursement at the time of printing. Bidders certify that prices were developed independently without consultation with other contractors.
